Job Summary:
We are seeking a compassionate and dedicated Registered Nurse to join our Home Care team on a PRN basis. Your primary responsibility will be to assess patients' in-home care needs, contribute to the care plans, and implement appropriate interventions to promote optimal health and well-being.

This is a flexible opportuity where you can determine your schedule for visits.

Duties:
- Conduct initial comprehensive in-home assessments of patients' health status and develop individualized care plans

- Conduct Routine Supervisory Visits minimum every 90 calendar days (in clients' home)
- Collaborate with interdisciplinary healthcare team members to ensure coordinated and effective patient care
- Document and maintain all patient interactions and assessments accurately and timely in electronic medical records

Requirements:
- Valid Registered Nurse (RN) license in the state of Virginia
- Minimum of 1 year of nursing experience preferred
- Experience in home health or a related field is a plus
- Proficient in medical record documentation and electronic medical records systems
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members

Job Types: Contract, PRN

Pay: $85.00 - $100.00 per hour
