Responsibilities:
- Perform weekly nursing visits (or more depending on plan of care) for adult patients in the home care setting.
-Monitor effectiveness of medication regimen in relieving pain and providing comfort
- Perform health coaching and education to patients and their families
- Collaborate with healthcare team members to develop and implement individualized care plans
- Administer medications via various routes, including oral, rectal, and intramuscular
- Perform catheterization procedures as needed
Requirements:
- Valid LPN license in the state of [location]
- Experience with inhalation therapy and the administration of respiratory medications
- Knowledge of infection control practices
- Ability to effectively communicate with patients, families, and healthcare team members
