Essential Functions
Note: The essential duties and primary accountabilities below are intended to describe the general content and requirements of this position and are not intended to be an exhaustive statement of duties. Specific tasks, responsibilities or competencies may be documented in the incumbent's performance objectives as outlined by the incumbent's immediate supervisor or manager.
1. Recommends goal directed medication therapy to reduce symptom burden.
2. Answers consults from care givers for hospice medication management.
3. Serves as subject matter expert (SME) for managing hospice medications.
4. Provides hospice clinical staff with education and training on medication therapy, as requested and assigned.
5. Evaluates drug therapy and communicates/documents findings and recommendations.
6. Demonstrates competency in equianalgesic dosing, generic and therapeutic substitution, formulary management and prior authorization procedures.
7. Demonstrates understanding, competency and adherence to all state and federal pharmacy practice rules and regulations.
8. Recommends medications, including initiation, continuation, discontinuation and altering therapy, based on established protocols and evidence.
9. Assures optimal, effective pharmaceutical care and collaborative practice for hospice patients.
10. Maintains a current knowledge of all professional and technical developments and requirements affecting the defined area of responsibility.
11. Prepares information for and attends IDG (Interdisciplinary Group) meetings when requested.
12. Processes admission, transfer, and discharge information by adjusting patient data in computer.
13. Maintains accurate patient medication lists during consultation.
14. Communicates effectively with non-clinical staff regarding billing functions.
15. Assists in maintaining extreme customer service and client relations.
Marginal or Additional Functions
1. Protects company's intellectual property and the privacy of customers' and patients' data
2. Performs other related duties as assigned to support achievement of Pharmacy goals and objectives.
Supervisory and Managerial Responsibility
• This position does not have supervisory or managerial responsibility.
Knowledge, Skills & Abilities
Education, Licensure or Certification:
• R.Ph required, PharmD preferred
• Full, current, and unrestricted license to practice pharmacy in the state of Pennsylvania and in the state in which incumbent resides
• Additional licensure may be requested pursuant to the needs of the organization
• Board certification in a relevant specialty certified by the Board of Pharmaceutical Specialties preferred; Required to attempt Board Certification within three (3) years of hire
Work Experience or Related Experience:
• Completion of a PGY-1 or PGY-2 Residency in Palliative Care or Pain Management or equivalent experience in Hospice/Palliative Care/Pain Management preferred
• Experience in collaborative development and launch of new pharmacy or patient care services, including development of policies, procedures, tools and metrics preferred
• Multidisciplinary team leadership experience in a variety of settings preferred
• Proven support level experience in Hospice preferred
• Experience with various web browsers (Google Chrome / Firefox), Google Docs, Google Sheets, Microsoft, or similar software required
• Familiarity with various video webinar platforms (GoogleMeet, Microsoft Teams, Zoom)

• Proficient typing and computer skills are paramount to employee satisfaction and success while fulfilling responsibilities

• Excellent verbal and written communication skills (typing)
• Excellent attention to detail
• Proven ability to manage multiple projects simultaneously while paying strict attention to detail
• Ability to maintain confidentiality, and to exercise discretion and good judgment
• Ability to move from one task to the next efficiently
Equipment:
• Working knowledge of a PC, business and communications software (MS Office, Google) and web-based tools are required

• Limited travel may be required

• This position entails a high level of work performed at a computer terminal using multiple monitors and telephone or headset equipment throughout the day
