Duties:
- Conduct comprehensive assessments of patients' health status
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor patients' progress and make necessary adjustments to care plans
- Educate patients and their families on managing their conditions and promoting wellness
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care
- Document patient information, treatments, and outcomes accurately and timely

Experience:
- Minimum of 12 months of Home health experience in past 3 years
- Strong clinical skills and knowledge of best practices in patient care
- Experience working with diverse patient populations, including children, adults, and seniors
- Familiarity with electronic medical records (EMR) systems
- Excellent communication and interpersonal skills

Requirements:

• Active License
• BLS
• COVID Card
