Careers at DHEC: Work that makes a difference!Embracing Service, Inspiring Innovation, Promoting Teamwork, Pursuing Excellence, and Advancing EquityIdentifies, research, and tracks health equity activities of external partners and community-based organizations that address the social determinants of health that influence health outcomes and health equity opportunities for vulnerable, under-served, and special populations. Develops partnerships and manages communication efforts with internal and external stakeholders to build health equity capacity across the organization. Represents the Office of Health Equity & Inclusion (HE&I) through regular attendance in internal and external statewide meetings to share healthy equity best practices, strategies, and resources. Supports the Multicultural Health Program Manager in identifying and researching ethnic and culturally diverse populations in South Carolina. Assists with collecting, tracking, and managing population health data on identified populations. Assist in developing and coordinating HE&I multicultural health initiatives. Collaborates and assists multi-disciplinary teams on existing and new public health programs to improve health equity outcomes for SC communities experiencing disparate health impacts. Provide programs insight on health equity, social determinants of health, and cultural diversity and inclusion. Coordinate and manage special projects, assignments and activities; collect, compile, analyze and monitor progress; present findings to leadership and community partners, identify alternatives and make recommendations. Maintains and disseminates HE&I or agency communications that impact external organizations, community leaders, and stakeholder to build partnerships, garner support, and coordinate initiatives aims at promoting health equity and inclusion. Develops mechanisms to track, store and report identified health equity trends, qualitative data, and internal partner's program activities. Manages Excel and/or SharePoint reporting and communicate project status by written or oral reports and presentations to stakeholders, supervisor, and agency executive leadership.
