Join the team at State Hospital South as a Psychiatric Treatment Coordinator. This position is primarily dedicated to assisting State Hospital Patients transition throughout the entire process: admission, treatment, benefit eligibility, to discharge. You will join a team that is passionate about working with individuals with mental health conditions, and serve in the critical role of discharge planning and coordination. You will have the opportunity to assist patients with social security and Medicaid applications as well as general case management, while being a part of an interdisciplinary treatment team, work in a challenging and exciting hospital environment. EXAMPLE OF DUTIES:

  • Be able to effectively communicate both verbally and written with patient's to include internal and external partners.
  • Treatment Team participation, including transcribing decisions made in treatment team.
  • Case management based on individual patient needs.
  • Assist patients with social security and Medicaid applications.
  • Work with Navigation Department on some discharge coordination.
  • Admission intake interviews, patient funds, coordinate discharge surveys, discharge appointments, and discharge charting.
  • Daily intensive contact with psychiatric patients, including the ability to assess and intervene as appropriate with mentally ill patients.
  • Role model and teach appropriate job skills to other employees.
  • Role model best practices related to reduction of seclusion and restraint.

MINIMUM QUALIFICATIONS:

You must possess all the minimum qualifications below to pass the exam for this position. Please make sure your resume or work history supports your meeting the minimum qualifications for this position. Failure to do this may disqualify you from being considered for this position. It is highly recommended to attach a one-page cover letter to your application to demonstrate how you meet the requirements below.

  • This position requires a Certified Nursing Assistant (CNA) certificate or higher (LPN, RN).
  • Good knowledge of therapeutic techniques used to treat mentally ill patients. Typically gained by successful completion of 1-3 college level courses (300 or 400 level) in the areas of psychology, social work, counseling, human behavior AND one year of experience in the mental health field utilizing therapeutic techniques in treating mentally ill patients. Experience must be in a controlled setting where you were responsible for implementing formal or prescribed therapeutic programs for others OR at least two years experience as described above.
  • Good knowledge of common mental illnesses and symptoms. Typically gained by successful completion of 1-3 college level courses (300 or 400 level) in the areas of psychology, social work, counseling or human behavior OR at least one year experience working in the mental health field in a position which required a good knowledge level of common mental illnesses, symptoms, and appropriate terminology as predominate responsibility of the job.
  • Good knowledge of civil and legal rights of institutionalized persons. Typically gained by at least three months experience working at the professional or paraprofessional level with institutional patients. Practicum, field work or internship experience may qualify OR through a college course of one, 4-6 hour seminar, workshop or program dealing with the civil and legal rights of institutionalized patients.
  • Experience implementing treatment plans. Typically gained by at least two years experience working in a position in the mental health field which required implementing treatment plans. This experience would include identifying and describing behavior, therapeutic record keeping and making written documentation, providing input to the treatment team when the plan is being developed, helping coordinate service delivery and evaluating the effectiveness of the plan.
  • Experience leading therapeutic activity groups. Typically gained by at least two years experience working in a position in the mental health field which required leading or assisting a therapeutic activity group as a regular part of your job responsibilities (1-2 times per month) OR by a six-month supervised practicum, internship or field work experience specific to working with therapeutic activity groups in the mental health field.

Below is preferred experience. It is not required for the position but applicants with this experience may receive consideration over other applicants.

  • Experience identifying needs of patients who would be discharging from the hospital. Typically gained through at least six months experience working in a mental health facility.
  • Some knowledge of assisting patients complete applications for benefits programs including Medicaid, Social Security Programs and Entitlements. Typically gained by at least six months of professional experience performing these duties.
  • Some working knowledge of assisting others with accessing social services and making appointments with community service providers. Typically gained by at least six months of professional experience performing these duties.
