Outreach Coordinator – CET

Seeking an Outreach Coordinator (CMA/RMA) in Johnson City, TN interested in joining our Clinical Excellence Team. Office hours are typically 8-5, Monday through Friday.

PRIMARY RESPONSIBILITIES:

• Assess the patient’s electronic medical record (EMR) and need for accessing the ER.
• Appropriate education delivered to patients regarding office hours and Walk-in Clinic/Urgent Care options.
• Communicate information to primary care providers of patients discharged from inpatient facilities.
• Coordinate activities for the Palliative Care Clinic and Home Visit Team.
• Assist with outreach efforts for other patient situations or initiatives.
• Manage/Maintain CareTrac System.
• Utilize/Maintain the Practice Management System.
• Manage/Maintain patients’ EMR.

REQUIREMENTS:

• High school diploma or GED and a CMA/RMA certification.

EXPERIENCE:

Five or more years of experience in a medical office or other healthcare environment. Previous experience with patient communication/outreach efforts is desirable.
