About the Division of Youth Services:

The Division of Youth Services (DYS) provides a continuum of residential and non-residential services that encompass juvenile detention, commitment, and parole. DYS is the agency statutorily mandated to provide for the care and supervision of youth committed by the District Court to the custody of the Colorado Department of Human Services. The Division operates numerous secure facilities that serve youth between the ages of 10 and 21, who are pre-adjudicated, sentenced, or committed. The Division also contracts with numerous private residential and non-residential service providers throughout the State. For pre-adjudicated youth, the Division is also responsible for the management and oversight of Colorado Youth Detention Continuum, a State-funded, locally administered program that provides services to youth at risk of further progressing into the juvenile justice system. In addition to residential programming, the Division administers juvenile parole services throughout the State which includes but is not limited to parole supervision through case management to ensure community safety.

Description of Job

Purpose of the Work Unit:
The purpose of this work unit is to provide a safe, secure, and healthy environment for all youth who are committed to the secure treatment facility. Services are provided consistent with Evidenced Based practices and the agency's Sanctuary model and trauma responsive environments. Services include transition programming to youth across the campus and collaborates with Behavioral Health Specialists and other members of the Multi-Disciplinary team. This program works to safely and efficiently treat and return youth back into the community by providing a trauma responsive environment while ensuring safety for youth, staff and the community.
The transition coordinator position is responsible for the successful integration of committed youth into the community. This position addresses the following needs: individual case management, employment, education, transportation, housing, job search, ID cards, birth certificates, Behavioral Health services in the community, mentor connections, faith based partnerships, community supervision when needed, and transition classes. Through collaboration with Multi-Disciplinary teams, transition plans are developed and services identified based on individual needs. The focus is to ensure successful reintegration of youth back into communities while promoting protective factors that increase support and resources. Collaborates with internal stakeholders as well as workforce development centers, community based organizations, schools and employers.
Duties:
  • Prepares and maintains a case folder for each assigned youth using Google folder or similar electronic method to store and share transition case folders with other members of the MDT/Transition Team.
  • Conducts initial interview and assessment of youth in transition within five working days of referral.
  • Discusses with youth how such issues as substance use disorders and anger management problems might play a role in their community activities and transition support needs.
  • Facilitates research-based life skills assessment and curriculum for transitioning youth, facilitating life skills group weekly for youth in transition.
  • In collaboration with the MDT, develops measurable transition goals based on the ITP objectives for caseload of youth on Transition Status.
  • Incorporates a review of youth risk/needs/responsivity requirements to build an effective transition plan, incorporating the Roadmap to Success and Transition Checklist.
  • In collaboration with Behavioral Health Specialist and program staff, assists youth with the completion of the youth portfolio.
  • Meets with youth to evaluate their progress in accomplishing goals and maintaining the terms specified in their transition plans.
  • Informs youth of the requirements of temporary release passes, such as safety plans, sponsorship, schedule and attendance documentation, paycheck management, restitution payments or educational and employment stipulations.
  • Completes all necessary transition paperwork for offsite events. Monitors, evaluates, and documents progress and communicates regularly with youth’s Behavioral Health Specialist and Client Manager.
  • Utilizes outside stakeholders and community resources to assist the family and youth to set up proper services for the youth’s parole.
  • This position is responsible for assessing a youth’s safety, based on collaborative and shared information with a youth’s MDT, as to readiness to go on passes in the community.
  • This position makes decisions regarding a youth’s need for support services and community transition needs.
  • This position is responsible for organizing their time to be able to attend MDT meetings for youth in transition, balanced with facilitating groups and taking youth on community passes. The position seeks the support and collaboration of their supervisor and Behavioral Health Specialists in this process.
  • Decides when to use Motivational Interviewing and other supportive communication techniques, as well as articulation of potential graduated responses for behaviors when working with youth to assist and motivate them.
  • Other duties as assigned.

Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

Minimum Qualifications:

  • 6 Years of professional experience in youth counseling or criminal justice with detained and/or adjudicated youth or an occupational field related to the work assigned.

~OR~
  • A combination of related education in the study of behavioral science, counseling, psychology, criminal justice, criminology, human services, social work, sociology, or youth and adult corrections, or similar field and/or relevant experience in an occupation related to the work assigned equal to six (6) years.

Preferred Qualifications:
  • Previous Client management or case management experience
  • Ability to establish collaborative relationships with internal and external customers, ie; contract program, state facilities, community providers, Courts, Community Review Boards, Parole Boards, and Law Enforcement agencies.
  • Experience implementing and utilizing DYS initiatives
  • Previous experience in program monitoring or evaluation
  • Collaborative work experience with diverse populations and organizations
  • Experience providing technical assistance and training

Conditions of Employment:
  • Be a minimum of 21 years of age, with no felony convictions or history of domestic violence
  • A pre-employment criminal background check will be conducted as part of the selection process as this position is deemed to have direct contact with vulnerable persons. Felony convictions, conviction of crimes of moral turpitude, or convictions of misdemeanors related to job duties may disqualify you from being considered for this position. Should your background check reveal any charges or convictions, it is your responsibility to provide the CDHS Background Unit with an official disposition of any charges. Additionally, you will be required take a drug screen.
  • Included in the background investigation is a current/previous employer check. Your current and previous employers within the last 10 years must be listed on your application even if the job duties are unrelated to Correctional Youth Security Officer I position. Incomplete employer information (address, phone, supervisor name) or omission of former employers may result in application disqualification.
  • Shift Work: Subject to assigned shifts
  • Must possess a valid, unrestricted U.S. Driver's License
  • Transportation of youth in state vehicles, travel to assigned trainings and/or alternate work sites.
  • TB (Tuberculosis Test)
  • Continued employment is contingent upon successful completion of the DYS Pre-service Training Academy, Field Training Program (FTO) and mandatory annual training in all subject areas (if applicable).
  • Former State employees who were disciplinarily terminated or resigned in lieu of termination must (1) disclose that information on the application (2) provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position, (3) provide the employee number from the applicant’s prior State employment. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination and failure to provide this information will disqualify the applicant from future State employment with CDHS.
