JOB TITLE: Site Coordinator

DEPARTMENT: Transitional Youth Services

REPORTS TO:  Program Manager

POSITION STATUS: Non-Exempt-Full-time-40 hours per week-100% On-Site

SALARY: $27-$31 per hour

DESCRIPTION:

PROGRAM DESCRIPTION:

StarVista’s Transitional Youth Services, THP-Plus Program is a residential program for youth ages 18-24 who have exited the Foster Care and/or Juvenile Justice system. The goal of the program is to appropriately prepare the youth for independent living and self-sufficiency. The program provides case management, advocacy/support, and independent living skills education to residents.

POSITION PURPOSE AND OBJECTIVES:

Provide goal-directed, case management services in residential and community settings, for transition-aged youth (18-25 years), formerly involved in Foster Care or Juvenile Probation. Participate with the Program Manager and Residential Team to provide ethically appropriate, theoretically sound, culturally informed, and holistic interventions to address client needs and enhance client strengths. Work with Program Manager and team to actively establish housing opportunities for youth. Conduct regular outreach to recruit eligible participants.

Requirements

ESSENTIAL JOB FUNCTIONS:

  • Oversee single-site facility where up to nine (9) transitional-aged youth reside
  • Work with the Operations team to ensure proper facility maintenance and upkeep
  • Assist staff prepare building for inspections, special events, etc., and ensure units are move-in ready, when applicable.
  • Manage residential budget, incorporating systems of accountability and documentation. 
  • Ensure necessary documentation and reporting are conducted in a timely matter including, but not limited to, meal planning, weekly allowances, and transportation funds. 
  • Establish rapport with residents and maintain a safe, secure home environment.
  • Provide on-site checks and monitoring of youth residents. Checks/monitoring include participation in the chore program, adherence to curfew times, and observance of the agreed-upon program policies.
  • Train and manage overnight/awake residential counselors who will be on-site.
  • Facilitate two (2), in-person, Overnight Staff meetings per month. 
  • Attend collaborative team meetings as assigned by the Program Manager.
  • Collaborate with colleagues to assess clients’ family, school, peer, and work environments and ascertain strengths and needs.
  • Provide case management to youth participating in the housing and case management program (THP-Plus and Aftercare) to support their transition to independent living.
  • Help youth establish and meet their Transitional Independent Living Plan (TILP) goals; aimed to target personal, financial, educational, relational, and employment-related objectives. Conduct assessments of client strengths and needs and advocate for them in the community.
  • Advocate for clients and help them navigate independently within the community.
  • Facilitate or co-facilitate psycho-educational youth groups
  • Understand independent living skills development, mental health and substance abuse issues, and education, employment, housing, and financial/budgeting needs.
  • Have a clear understanding of adolescent development and issues specifically related to the lives of youth in foster care and transition-aged youth. 
  • Encourage youth to develop healthy relationships and assist them in relationships with family and significant others.
  • Perform outreach in the community and work closely with county and community partners.
  • Maintain consistent, confidential case records and supporting documentation.
  • Participate actively in quality assurance and program evaluation efforts.
  • Report any violations of the THP-Plus Program policy to Program Manager or Site Coordinator.
  • Manage crises or emergency situations as they arise.
  • Provide support to the Program Manager as needed.
  • Attend All Staff meetings, mandatory training, and client activities as needed.

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:

  • Demonstrable and verifiable clinical skills and experience in providing direct service to clients including assessment, individual case management, group psycho-education, and harm reduction interventions.
  • Strong ethical principles reflected in practice, including a thorough understanding of client confidentiality requirements.
  • Excellent understanding of issues related to diversity, equity, and inclusion, especially within foster care and/or juvenile justice settings. 
  • Ability to provide culturally informed services and commitment to the continuous exploration of these issues with clinicians. Understanding of diversity issues, willingness to explore self in relation to these issues, and the ability to provide culturally informed therapeutic services to a diverse clientele.
  • Strong organizational and communication skills, sound judgment, and the ability to set healthy boundaries
  • Ability to function as a member of a team with diplomacy and professionalism.
  • Ability to work as a team member when necessary to accomplish tasks supporting the agency’s mission.
  • Experience working with youth in Foster Care

MINIMUM QUALIFICATIONS

  • Valid CA Driver’s license, a working car, proof of car insurance, and the ability to pass fingerprint and TB clearance is required.
  • 2 years of verifiable experience working with at-risk youth and/or young adults
  • Experience working with youth in Foster Care
