POSITION SUMMARY: 

This position is part Direct Support Professional and part Medical Support Team Member.

Medical Support Team Member:

This position schedules and attends exams for those we serve, which are as follows: Annual Complete Physical Exam, Annual Eye Exam, and Annual Dental Exam, as well as Specialist visits per MD order or ID Team requirements. Schedules any follow-up visits if indicated. Schedules and procedures, labs, tests, or other medical requirements. Transport individuals we serve with or without adaptive equipment in their own or company vehicles as necessary to appointments. Assures that orders/recommendations are carried out accurately and promptly. Assures there is a written report on every Doctor’s visit, and logs are filled out with results and recommendations for any medication changes or other MD orders. Assumes a supportive role and is responsible and accountable for facilitating, communicating, and collaborating with the healthcare team so we serve and their families. Promotes the optimal health, well-being, and safety of those we serve. Responsible for accurate chart documentation and medical record maintenance.

Direct Support Professional:

The primary responsibility of the Direct Support Professional is to ensure the health and safety of those supported. DSPs are responsible for providing support as identified in the Individual Support Plan, collecting data, and documenting the provision of that support. Direct Support Professionals support individuals receiving services in their homes, place of work, community support, or day habilitation sites following the Individual Service Plans (ISP), subsequent protocols, and other plans. Provide training and support in areas such as self-help skills, safety skills, hygiene skills, medication skills, food/meal skills, money skills, housekeeping skills, community skills, social skills, vocational skills, safety skills, communication skills, and healthy living skills.

Requirements

SKILLS & EXPERIENCE (These are the education and experience requirements.)

· High School diploma or equivalent. 

· Computer experience with Word and Excel. 

· Starpoint vehicle insurance approval, certificate of personal auto insurance, and reliable transportation.

· CPR and First Aid. 

· Cell phone with texting ability.

· Experience working with DD individuals preferred; Experience working in a clinical setting (nursing home, MD office, or hospital) could be considered a substitute).

· Ability to work independently.

· Must demonstrate organizational and time management skills.

· Must have practical verbal and written communication skills.

· Must be able to work with clients, staff, parents, providers, and community representatives.

· Must be able to work under stress and meet deadlines.

· Must be able to work with minimal supervision.

· Must coordinate with families, supervisors, healthcare providers, and those we serve pleasantly and effectively.

· Must exhibit mature judgment. 

ESSENTIAL FUNCTIONS:

They might be required to carry 50 pounds, push/pull 100 pounds, lift and carry heavy objects such as wheelchairs and boxes of supplies, stand 4 hours per day, walk 4 hours daily, etc.
