POSITON SUMMARY:

The Wraparound Coordinator will assist with program development, marketing, staff training and service monitoring for the clinic’s Wraparound program. This position will take the lead role in coordinating the activities of the Wraparound team as well as collecting and reporting all necessary data to the supervisor. This position will maintain a caseload and is responsible for developing and coordinating strength based, family centered teams to meet the individualized needs of children and families in accordance with the Wraparound Model.

EDUCATION AND EXPERIENCE:

  • Must have a minimum of a bachelor’s degree in human services or a related field is required.
  • Must meet the qualifications of a child mental health professional as defined by the Michigan Department of Community Health.
  • A current State of Michigan Social Work registration is preferred.
  • Must have a minimum of 3 years supervised experience in the examination, evaluation, and treatment of minors and their families.
  • Must be CAFAS/PECFAS certified reliable rater or attend and complete the CAFAS/PECFAS training within the first 3 months of hire.
  • Must have a working knowledge of the Wraparound Model.

DUTIES AND RESPONSIBILITIES:

  • Will complete administrative tasks assigned at the discretion of the Wraparound Supervisor such as, but not limited to, the organization of referrals, staff assignment, and data collection.
  • Will provide direct service to clients and families.
  • Will have at least weekly contact with the child and family and/or members of the Child and Family Team regarding the implementation of the Wraparound Plan.
  • Must assemble a Child and Family Team within two (2) weeks of enrollment by interviewing the family, identifying family members/natural supports/agency representatives and other significant persons.
  • Will convene the Child and Family Team and guide its members throughout the Wraparound process, ensuring adherence to the model fidelity and Wraparound Values.
  • Will assist each child, youth, and family in defining the frequency of child and family team meetings, the standard for meeting frequency should occur at least weekly/every other week until the family begins to transition toward graduation. Most families will need meetings weekly in the first few months due to safety risk and high needs. Once the transition/graduation plan is developed, the meetings should then occur at least monthly for no longer than 3 months.
  • Must work with the family to uncover youths/family’s strengths and needs and provide assistance with any immediate needs and complete the initial strengths assessment and needs and priorities within thirty (30) days of enrollment.
  • Must work with the family and submit a comprehensive Support Plan (proactive and reactive crisis/safety plan) within 1 week of enrollment. The plan must reflect the best possible fit with strengths, culture, families, and beliefs of the family.
  • Must work with the Child and Family Team and submit an Individualized Plan of Care (POC) with services and resources that are community based and culturally relevant within 30 - 45 days of enrollment and an updated plan every 90 days thereafter.
  • Ensures the needs and services identified by the Child and Family Team are met.
  • Ensures community resources are sought first with the assistance of the Child and Family Team and the Community Team.
  • Collaborates with other necessary individuals the youth and family has contact with, such as, CMO Workers, Teachers, Judges, District Attorneys, Mental Health Clinicians, Physicians, etc. This means facilitators maintain frequent contact, invite them to Child and Family Team meetings with adequate notice and provides copies of the Plan of Care within 1 week of completion to team members.
  • Ensures all barriers related to Plan implementation are addressed.
  • Makes family aware of Wayne County SOC activities and encourages youth and parent involvement in leadership and advocacy within the SOC.
  • Will facilitate the necessary referrals, information, and cooperation with collateral resources to ensure quality care and service delivery.
  • Monitor the provision and quality of services provided to the family and is the liaison when new services/resources need to be sought or developed.
  • Complete all the necessary paperwork in a strength-based manner per Wraparound/Agency requirements.
  • Develop/maintain liaison relationships with various community agencies, groups/volunteers and key stakeholders.
  • Assist in advocacy for the development and provision of appropriate community resources and services for the client/family.
  • Facilitate alternative placement of a client when necessary.
  • Participate in and contribute to staff case reviews and staff training in collaboration with mental health therapist when appropriate.
  • Cooperate in any investigation related to personnel, licensing, accreditation, or other circumstances.
  • Responsible for working with team members, supervisor, child, families, and community contacts in a manner that is conducive to the philosophy and mission of the program and agency.
  • Attend mandatory agency meetings and trainings.
  • Must participate in a minimum of 24 hours of child and adolescent specific training annually.
  • Provides or secures support and crisis/emergency services for the youth/family. This may be done through face-to-face contact, phone contact or availability by beeper or an on-call system.
  • Contribute to the building and maintenance of the community team as directed by supervisor and program manager.
  • Must be able to prepare for and testify in court.
  • Will provide transportation for appointments, crisis/respite services, etc. for clients, when necessary.
  • Responsibilities related to the wraparound docket:
  • Represents the Child and Family ensuring family and youth voice informing the court of the strengths, needs, outcomes and the work of the Child and Family team.
  • Represents the Child and Family Team during the court process to enhance the awareness of needs driven outcome based, community-based interventions.
  • Core Competencies:

  • Training:
  • Must complete the MDCH 3 Day Wraparound Training within the first 3 months of hire.
  • Attending ongoing training on variety of topics ensure quality care to families related to the provision of Wraparound; including at least 1 quarterly training that is Wraparound specific.
  • Agrees to comply with all policies and procedures of Starfish Family Services.
  • Works flexible hours to accommodate the needs of the program and clients.
  • Meets the productivity expectations as assigned by program supervisor.
  • Works cooperatively as a member of a team with program staff and community resources.
  • Responsible for keeping client files in order and up to date.
  • Assist in finding and directing clients in case of an emergency such as a fire or tornado.
  • Perform other reasonably related duties as assigned by immediate supervisor and other management as required.

  • Must have a working knowledge of the psychological and physical effects of incest, violence, abuse, and neglect in a family.
  • Must be able to develop and carry out a plan of service based upon the above knowledge.

  • Ability to use standard office equipment including a computer, navigating the internet, experience with electronic records and the use of Microsoft Word.
  • Ability and willingness to work with all members of the community regardless of race, gender, age, cultural, or ethnic background.

  • Understanding of how to locate and access a range of community resources including, but not limited to low-income housing, shelter, transportation, employment and health care.

LICENSING AND OTHER REQUIREMENTS: 

  • Must possess and maintain a current, valid State of Michigan Chauffeur’s license and automobile insurance.
  • Must have reliable transportation to be able to meet with clients outside of the clinic setting.
