Description
StarCare Specialty Health System (StarCare) is seeking a full time PCA/Transportation Supervisor to join its outstanding team in the Silver Star/PACE program of all-inclusive care for the elderly.
Responsibilities:
  • This position is responsible for the transportation program, including developing the transportation schedule and coordinating transport of participants, meals, durable medical equipment, pharmacy, and any other transportation as deemed necessary. Responsible for hiring, training and supervision of all Transportation/Personal Care Attendants. Functions as a member of the Interdisciplinary Team (IDT). Maintains regular attendance at, and participates in IDT meetings; communicates participant changes, ensures adequate staffing and improves efficiency.
Essential Duties:
  • Manages workflow and training of new hires on how they can best serve the Silver Star Participants and develops team cohesion.
  • Collaborates, participates, and communicates with Interdisciplinary Team (IDT), physicians, personal care attendants and other related staff
  • Ensures deadlines and goals are met to prevent gaps or downtime while giving direction and feedback about what needs improvement.
  • Completes or delegates participant's transportation and PCA assessments and reviews in IDT meetings.
  • Completes required documentation of services, including service logs, PayCom, mileage logs, Vehicles accident reports, educates on Q-straints, incident reports, and other related documentation.
  • Identifies patient service requirements by establishing personal rapport with potential and actual patients and other persons in a position to understand service requirements.
  • Assures quality of care by developing standards of care, enforcing adherence to center of medication and Medicaid agencies requirements and to other governing agency regulations, measuring health outcomes against standards, and making or recommending.
  • Hires, onboards, directs, mentors, develops, and schedules staff, including communicating job expectations and providing performance reviews.
  • Performs duties as assigned.
Schedule: This is a full-time/exempt position scheduled Monday - Friday, 8am - 5pm, with flexibility to meet the needs of the department.

Qualifications
Qualifications:
  • A bachelor's degree from an accredited four (4) year college or university
and
  • two (2) years full-time related experience.
  • Preference will be given to applicants with at least 2 years of experience in transportation, fleet management and/or operating accessible vehicles.
  • Supervisory experience is preferred
  • Must have at least one year experience with a frail or elderly population, meet the standardized set of competencies for the specific position description before working independently, and be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
Skills & Abilities:
  • Ability to assist with personal care in participant's home and/or Center sites, including wheelchair and ambulatory transfer between home, van and Center and van is required.
  • Ability to organize and prioritize a variety of assignments and manage time effectively.
  • Ability to communicate, both verbally and written. Ability to actively listen, manage time, and make decisions.
  • Ability to follow brief oral and written instructions and work under minimal supervision
Additional Requirements:
  • Must pass a pre-hire drug screen and criminal background check.
  • Must have a valid Texas driver's license, an excellent driving record, and be insurable under the Agency's insurance.
