StarCare Specialty Health System (StarCare) is seeking a full time Bachelor Level Social Worker to join its outstanding Aging and Disability Team at Silver Star. This role performs the completion of case management duties in order to promote participant’s social and emotional well-being to ensure participant is living safely in the community.

Responsibilities:

• Provide hands-on support and crisis intervention
• Address living arrangements, money management and food insecurity
• Coordinate community resources for participants on assigned caseload
• Assist participants with paperwork/applications as needed
• Communicate pertinent information to participant’s interdisciplinary team
• Complete documentation and related reports

Essential Duties:

• Duties include but are not limited to housing assistance, money management, addressing food insecurity, and completion of advanced directives, applications and documentation.

Schedule: This is a full-time/exempt position scheduled Monday - Friday, 8am - 5pm, with flexibility to meet the needs of the department.

Qualifications

Qualifications:

• Bachelor's degree in social work
• licensure (preferred but not required).

The qualified candidate will:

• have at least one year experience with a frail or elderly population,
• meet the standardized set of competencies for the specific position description before working independently, and
• be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact

Skills & Abilities:

• Knowledge of community resources, crisis intervention and case management services is also preferred.
• Knowledge and skill in operating standard office equipment (phone, fax machine, copier, printer, pager, etc.) and personal computer with Microsoft Office Products is required.

Additional Requirements:

• Must pass a pre-hire drug screen and criminal background check.
• Must have a Texas driver's license and be insurable under the Agency's insurance.
