Job Shift: Shift 1
Shift Details: 7:30 am to 3:30pm Monday thru friday

St. Vincent's Medical Center in Bridgeport has more than 3,200 employees. It includes a 473-bed community teaching hospital, a 76-bed inpatient psychiatric facility in Westport, a large multispecialty provider group, and special needs services for adults and children. St. Vincent's is the first hospital in Fairfield County to be integrated into the Hartford HealthCare network, and is the system's second-largest hospital.
Job Summary: Provides informal training and direct care to clients based on person-centered plans, individual needs/requests and daily household operations.
Responsibilities:
  • Supervises, trains, and/or assists client in accordance with the person-centered plan, including personal care, recreation, and reinforcement of individual habilitation objectives.
  • Assures client's personal property and adaptive equipment is clean and in good order including wheel chair, clothing and living space. Plans, schedules and transports clients to work, appointments and community activities.
  • Documents routine medications and treatments as prescribed by physicians and/or health care professionals.
  • Assists individuals with meal preparation and encourages good nutritional habits and adherence to prescribed diets.
  • Effective oral and written electronic communication skills.
  • Adapts positively to a fast paced changing environment.
  • Maintains required licenses, certifications and trainings.
  • Assists in all duties essential to the operational needs of site.
  • Completes all related paperwork or mandatory compliance with DDS and other agencies.
  • Assists persons served to meet their physical, emotional, spiritual developmental health and safety needs.
  • Other Duties as assigned

Qualifications

Licensure / Certification / Registration:
  • Required Credential(s):
  • Licensed Drivers License credentialed from the Connecticut Department of
Motor Vehicles obtained prior to hire date or job transfer date.
Work Experience: 1 year experience working with individuals with disabilities or completion of Human Services training preferred.
