St Johns Community Health is Hiring a Re-Entry Program Assistant Near Los Angeles, CA

The Program Assistant (PA) will provide support to the Program Manager (PM) assisting in supervision of RISE staff. The PA will also create and maintain program files and record, attend program related meetings and events in place of the PM. The PA may perform additional duties as directed by the PM. 

This position is grant funded. Scheduled to expire June 30th, 2026. Employment beyond the grant expiration date is not guaranteed. Should the grant not be funded, you are eligible and encouraged to apply for another position within the agency.

Qualifications/Licensure:

  • Employees are responsible for maintaining individual certifications.
  • CA Driver’s License 
  • Bilingual English/Spanish 
  • Self-Motivated 
  • Must have experience working with diverse populations. 
  • Must have experience working with incarcerated populations. 
  • Familiar with MS word, Excel, and Power point (preferred) 
  • Maintains personal appearance when interacting with patients, staff, and physicians. 

ESSENTIAL DUTIES AND RESPONSIBILITIES 

  • Provide support to Program Manager 
  • Monitoring the CHWs performance in their daily operation.
  • Ensure programming contractual agreements.
  • Coordinate with County staff training
  • Coordination with county staff on data collection, reporting and program analysis.
  •  Manage recruitment, interview, hire and onboarding of staff including CHWs and PSS’.
  • Create and maintain program files and records.
  • Attend program related meetings for or in place of program manager. 
  • Assist production of required weekly, monthly, and/or quarterly data collection reports; 
  • Maintain outreach activity calendars and logs according to program standards; 
  • Assist with program evaluations and updates to assigned program and services; 
  • Attend appropriate community resource meetings and trainings, as assigned; 
  • Work in collaboration with other departments when required; 
  • Attend meetings, trainings, and other work-related events as needed; and
  • (EXEMPT ONLY) As an exempt position, evening and/or weekend hours may be required to ensure the fulfillment of all duties and responsibilities; and 
  • Other duties may be assigned or may be modified as business needs dictate.
