Responsibilities

The Wound Healing Registered Professional Nurse is responsible and accountable for the delivery of patient care utilizing the Nursing Process. She/he implements the Nursing Service standards of care and practice, as appropriate to the ages of the patients served and the areas of clinical practice. The Wound Care Nurse assesses, treats, and creates care plans for patients with complex wounds. Wound Care Nurses demonstrate a high level of knowledge and expertise in the field of complex wound assessment and management. Wound Care Nurses care for and treat all types of complex wounds, acute and chronic. Among the most commonly treated wounds are those sustained from surgical wounds, diabetic wounds, venous stasis ulcers, arterial ulcers, radiation burns and pressure ulcers. Duties include: utilizes the nursing process in the systematic assessment and formulation of individualized plans of care to meet patient s needs. Exhibits critical thinking in the assessment, planning, prioritization, delivery and evaluation of patient care. Documents data collection and assessments plans of care interventions and patient responses per policy. Identifies individualized patient/significant other learning needs and implements appropriate measures to meet needs. The wound care nurse must be proficient in the staging and documentation of all complex wounds along with measuring, tunneling and undermining. The Wound Care Nurse must be able to accurately assess and document on the amount of granulation and fibrin noted in the wound bed along with the peri-wound appearance. The Wound Care Nurse must be proficient at assisting the physician with all debridement techniques, surgical and otherwise as well as any restorative procedures. The Wound Nurse is trained in the application of, and has an understanding of the role of wound dressings such as absorptive, alginates, antimicrobial, collagens, contact layers, foams, honey, hydrogels, and wound vacs, along with advanced treatment such as Apigraf and Oasis in treating wounds. Demonstrates knowledge, respect and sensitivity of patient's rights. Serves as a patient advocate. Assures confidentiality of patient information. Collaborates with other health care disciplines to coordinate patient care activities and discharge plans. Maintains effective communication with staff. Physicians, patients and families, also within the unit and with other departments. Coordinates and directs patients care activities, delegates task appropriately and accepts the accountability of her/his professional role. Participates in the development of standards and unit departmental performance improvement. Activity. Provide safe and therapeutic environment for patients that promote health. Makes decisions within her I his scope of practice and consistent with hospital policy, procedures and protocol. Participates in the orientation of personnel to the unit. Collaborates with the Nursing Leadership to identify staff learning needs and supports an atmosphere conducive to the development of all unit personnel. Utilizes opportunities for clinical learning and participates in continuing education activities to meet established requirements and for own professional development. Serves on hospital and departmental committee. Other related duties as required.

Qualifications

Bachelor’s Degree in Nursing preferred. Currently licensed and registered as a Registered Professional Nurse in New York State. A minimum of 1-2 years Wound Care Nursing experience required. Demonstrates good verbal and interpersonal skills.
