Responsibilities

The Patient Navigator will be responsible for providing client centered support, education, and linkage to care services to patients in the hospital’s Emergency Departments. Through a collaborative effort with the hospital’s existing HIV/Hepatitis C treatment & Prevention services and/or community service providers the Patient Navigator will coordinate and advocate for services that will promote positive health outcomes.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Conduct in-person outreach, education, and linkage to care services to individuals presenting at the hospital’s Emergency Departments at Andrus & Dobbs Ferry Pavilion. Support staff and patients to engage in HIV/HCV/HBV testing while receiving services by the hospital’s Emergency Departments. Offer supportive counseling and linkage to services as it relates to new HIV / HCV diagnosis, disease progression, available medical treatment, adherence, and other information as requested/needed. Provide patient centered referrals and follow-up to appropriate resources based on the patient’s HIV, HIV prevention, Hepatitis B and C health and/or behavioral health needs. Use available prevention, primary/specialty health care & behavioral health services to increase the number of people with a diagnosis of HIV and/or Hepatitis C who are linked to services. Develop relationships with internal and external service providers to ensure timely access to services for patients. Monitor and track all referrals, remind patients of upcoming appointments, (via phone call or text), verify that scheduled appointments were kept, and assist with rescheduling as needed.

 Develop a Brief Action Plan with patients to support engagement in self-management skills. Assess patient’s risk factors and implement a harm reduction approach to advocating for services. Responsible for documenting all interventions conducted with the patient or on their behalf in the hospital’s EMR (Meditech) in a manner that is timely, accurate and error free. Track all interventions on the program specific patient contact log. Documentation is to be completed immediately following the intervention and program specific monthly monitoring data is to be submitted to Project Coordinator no later than the 5th of the following month.Ensure that patient’s confidential health care and personal information is protected at all time. Responsible for ensuring that appropriate consent forms are signed and updated as required. Ensure that exchanges of information only occur with those individuals and programs for which written patient consent has been obtained. Responsible for the safeguarding of electronic devices and passwords. Work as a team player in all areas to enhance the program. Hear staff and patient concerns in an open and solution-oriented manner. Provide effective customer service as well as culturally and linguistically appropriate care to all customers (internal and external). Independently travel between hospital locations and surrounding community, as needed.

Qualifications

QUALIFICATIONS1. Applicant must have at least 12 months’ experience in patient navigator or similar work.2. Education/experience/personal knowledge relating to HIV, Hepatitis B/C, substance use, harm reduction.3. Specific language skills may be required. For example, some positions may require that the Patient Navigator be fluent in a language such as Spanish. 4. Computer operations5. Possesses well-developed and effective interpersonal skills and can communicate effectively verbally and in writing.6. Demonstrates confidence in actions and exercise good judgment.7. Possesses the ability to plan, organize, develop, and implement goals, objectives, policies, and procedures necessary for quality care.8. It is essential that this individual demonstrates the ability to recognize problems, approach them in an objective manner, and work collaboratively toward solutions.9. Valid driver’s license required.10. Access to personal car for work-related travel required.
