Candidate would need to be able to work various shifts, work weekends and holidays, possess a professional appearance, caring and compassionate demeanor, as well be attentive and observant.

General Purpose

Responsible for the overall planning, execution, and managing of nursing care of the skilled nursing unit and collaborates with the Assisted Living Manager regarding nursing care and services to residents of Assisted Living (including the Memory Unit). In addition, under the general supervision of the Assistant Executive Director, coordinate and supervise all the allied health professionals providing care and services in the health center.

Customer Service: Responsible for effectively meeting customer needs by caring out established customer service / guest relations protocols of SGV. Meeting the goal of having a very high degree of responsive and friendly customer service for all customers and taking responsibility for customer satisfaction.

Safety: Follows departmental safety practices. Works safely and contributes to a safe work environment for co-workers. Reports unsafe conditions and corrects in a timely manner.

Essential Duties

  • Coordination of resident care.
  • Responsible for managing/supervising nursing staff for resident care of the Skilled Unit.
  • Provides the delivery of professional nursing care in accordance with state regulations to ensure compliance.
  • Continuously assess through QAPI process to establish and employ best practices (clinical care and policy development)
  • Guides the Interdisciplinary Team in care plan development and execution for each of their respective disciplines.
  • Evaluation of new residents for admission to the SNU with coordination of the plan of care process.
  • Communicates with other healthcare providers, including therapy, pharmacy, hospice, lab and private caregivers for continuity of resident care.
  • Participation on community committees and community programs for enhancement of quality life for residents.

Administration:

  • Collaborates with Assistant Executive Director, Medical Director, Assisted Living Manager and medical staff in planning and developing administrative and operational policies and procedures.
  • Direct supervision and management of nursing staff providing skilled care in compliance with state regulations.
  • Collaborate with Independent Living Wellness Nurse and Assisted Living Manager regarding medical concerns within the community.
  • Ongoing evaluation of clinical competency of staff rendering resident care.
  • Monitors and evaluates overall resident care.
  • Evaluates educational needs of staff with provision of training as indicated. Develop and implement required in-service programs and ensure compliance.
  • Develop and promote effective professional working relationships with hospitals and community agencies.
  • Participates in budget planning for St. George Village Skilled Nursing Unit and ensures nursing budget goals are met.
  • Conduct audits and monitors nursing documentation to ensure compliance with regulations.
  • Participate in assigned facility committees related to healthcare.
  • Physician/resident/family liaison regarding resident care.

Nursing:

  • Develops and Monitors individualized clinical plan of care of residents per regulatory guidelines
  • Establishes and/or monitors policies and procedures related to the administration and control of medications under the direction of a pharmacist.
  • Advocate for the protection of resident rights under the guidelines of the Resident’s Bill of Rights.
  • Responds to various departments requiring emergency assistance and assigns staff accordingly.

Supervision:

  • Directly supervises processional staff on skilled nursing unit.
  • Fulfills supervisory responsibilities in accordance with organizational policies and applicable laws.
  • Conducts employee evaluations in accordance with agency policies and applicable laws.
  • Mentors and trains new hires as needed.

Residence Rights:

  • Provide quality nursing care to all residents in an environment which promotes dignity, freedom of choice, and individuality.
  • Orient new residents and family to the facility and its environment demonstrating interest and concern for their well being.

Safety:

  • Assure that staff is fully trained in the specific procedures related to all aspects of safety on the skilled nursing care unit.
  • Monitor daily activities to insure that proper safety procedures and being followed.
  • Knowledgeable of federal, state and local regulations related to skilled nursing and assisted living.

Qualification

Education and/or Experience: Level 7: Fifth year college or university program certificate; or two to four years related experience and/or training; or equivalent combination of education and experience.

Language Skills: Level 5: Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to write speeches and articles for publication that conform to prescribed style and format. Ability to effectively present information to top management, public groups, and or boards of directors.

Mathematical Skills: Level 4: Ability to work with mathematical concepts such as probability and statistical inference, and fundamentals of plane and solid geometry and trigonometry. Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.

Reasoning Ability: Level 4: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Certificates, Licenses, Registrations: Currently licensed in good standing as a Registered Nurse; Gerontology Certification a plus. Bachelor’s degree in nursing with at least five years related experience preferred

Additional Information

Working knowledge of computer software for spreadsheet and word processing. MDS experience is preferred. Experience in rehabilitative and/or Gerontology relations skills. Possess decision-making, assessment, and inter-personal relations skills. Familiarity and working knowledge of all federal, state and local regulations related to skilled nursing and assisted living.

The work week is generally 40 plus hours per week on a year-round basis: however, the position requires a willingness and flexibility in work hours when necessary to fulfill job responsibilities, including occasional evening or weekend work. Employee is subject to personnel policies and procedures prescribed for all personnel. Manage only 30 beds as a private pay skilled nursing.
