The Medication History Pharmacy Technician improves medication safety by assisting in the collection of an up-to-date medication list for patients admitted to the Hospital. Accurately obtains medication histories upon patients' admission, enters medication histories into electronic database, and performs all basic pharmacy technician functions in a timely manner.
Minimum Qualifications
- High School graduate or equivalent
- Fundamental understanding of personal computers, keyboarding skills, and familiarity with the Windows operating system
- Completion of one of the following:
- Current Pharmacy Technician certification
- Current American Society of Health-System Pharmacists (ASHP) or Pharmacy Technician Certification Board (PTCB) Medication Reconciliation Certificate or attainment within 6 months of employment.
- At least 3 years of pharmacy technician experience in a hospital or 2 years in a retail setting.
- Five years of experience as a pharmacy technician with a combination of retail and hospital experience; experience completing home medication lists as a medication reconciliation/history pharmacy technician.
- Completion of a pharmacy technician training program.
