Summary

According to social work practice and professional standards, works as a member of the healthcare team to facilitate continuity of care by assessing, planning, and coordinating discharge planning appropriate to patients’ level of care. Provides supportive counseling, crisis intervention, financial assistance and information, and referral services to both internal and external customers.

Minimum Qualifications

1. Master’s degree in social work or social welfare
2. Must meet one of the following
   1.  Licensed Social Worker (LSW) in the state of Pennsylvania or license eligible; must obtain license within three months of hire.
   2.  Licensed Clinical Social Worker (LCSW) in the state of Pennsylvania or license eligible; must obtain license within three months of hire.

Preferred Qualifications:

1. Minimum of one to two years of experience in a behavioral healthcare hospital setting.
2. Basic computer skills and proficiency in Microsoft Word.
