This physician role includes providing care for patients referred for specialized internal medicine care, care for patients who are referred to the Post-Acute Care clinic for medical issues after discharge from hospitals or emergency departments, and medical care and management for patients receiving skilled level care at local skilled nursing facilities (SNFs). POSITION OVERVIEW: The Post-Acute Care Internal Medicine Specialist Physician provides consultative specialized internal medicine care for patients referred specifically for management of complex medical issues such as hypertension, diabetes, hyperlipidemia, as well as pulmonary, renal, liver, and cardiac diseases that may not require subspecialized pulmonology, nephrology, hepatology or cardiology care. For patients who require close follow up after a hospitalization or emergency room visit, the Physician provides clinic-based medical care in the Post-Acute Care clinic as a transition from their inpatient or emergency focused on the patient’s acute medical issues that led to their hospitalization or ED visit. The Physician may also provide medical care as the attending of record for adult patients (minimum age 18) receiving care at local SNFs. The Physician responds to admission requests at SNFs in partnership with the other St. Charles Post-Acute Care SNFist providers and assumes care at such time that patients are transferred to the skilled nursing facility. This Physician works in a collaborative team with the Post-Acute Care clinic staff, the Post-Acute Care operations team, and other Post-Acute Care practice physicians and advance practice providers. Work schedule: 40 hours of direct patient care per week. Call expectation: None. Work location: The Physician will see patients primarily the Post-Acute Care Clinic, which is currently located on the Bend campus. The Physician may at times be asked to see patient at Regency Pilot Butte Rehabilitation Center for SNF patient care, and may be asked to see patients at other SNFs in the Bend and Redmond area from time to time. Other outreach locations may be added as patient and business needs dictate. ESSENTIAL FUNCTIONS AND DUTIES: (May perform additional duties of similar complexity within SCHS as required or assigned) Clinical Expectations: Provide high quality medical care for patients needing internal medicine specialist care, for patients referred to St. Charles Post-Acute Care practice for post-discharge medical care, and for patients receiving skilled level care at local skilled nursing facilities in an admitting role. Share in equitable distribution of work between all Post-Acute Care providers. Maintain credentialing and privileges to provide medical care for patients with complex medical needs in the Post-Acute Care Clinic or at SNFs. Collaborate with other providers within the Post-Acute Care division to ensure patients receive needed medical care to address their acute medical needs. Maintain familiarity with the St. Charles organization structure, communication systems, billing process, and completing billing and coding documentation within time assigned by St. Charles administration. Obtain medical knowledge and current skills through regular continuing medical education (CME) activity. Abide by all medical staff bylaws, rules and regulations. Maintain punctual presence for assigned workdays, in compliance with the SCHS policy on attendance. Keep medical knowledge and skills current through regular CME activity. Attest to having obtained 60 hours of Category 1 continuing medical education (CME) relevant to the practitioner’s current medical practice over the prior two-year period. Participation in quality efforts in partnership with St. Charles hospitals or divisions related to Post-Acute Care and at local SNFs may be requested from time to time. Practice and role model excellence in the Internal Medicine domains of diagnosis, aligning treatment goals to patient values, definitive treatment, supportive care treatment, follow-up care, promotion of good health and prevention of illness. Practice medicine within the scope of license, training and credentialing. Cooperate with other SCHS providers and caregivers to insure optimal patient care. Document all patient care in the proper form and in a timely manner in compliance with hospital policies and regulatory agencies. Administrative Expectations: Document all patient care within the Electronic Medical Record (EMR) and complete all necessary tasks in compliance with health system policies and regulatory agencies to ensure timely and accurate billing. Be familiar with the SCHS organization structure. Participate in SCMG physicians meetings and assist in policy formation for SCMG and Physician’s practice. Keep medical knowledge and skills current through regular CME activity. Must attest to having adequate (CME) to meet maintenance of board certification and compliance with MSO rules and continuing privileges without restriction. Keep all licenses and appropriate certifications current and unrestricted. Maintain active privileges at a St. Charles Health System hospital sufficient to permit performance of job duties. Collaborate with referring providers including hospitalists, emergency room providers, community primary care providers, SNF caregivers and leaders, other community providers caring for SNF patients to ensure communication with patients’ care teams along between different levels of care and strong partnerships with direct stakeholders in post-acute patient care delivery. Document all patient care within the Electronic Medical Record (EMR) and complete all necessary tasks in compliance with health system policies and regulatory agencies to ensure timely and accurate billing. Participate in provider and practice meetings and assist in policy formation for St. Charles and Physician’s practice. Keep all licenses and appropriate certifications current and unrestricted. This includes introducing oneself and stating your purpose, asking the patient their preferred name, spending time with the patient to determine their care goals, active listening, and using skillful communications (appropriate touch, eye contact, etc.). Complies with all applicable laws, regulations, policies and procedures, supporting the health system’s corporate integrity efforts by acting in an ethical and appropriate manner, reporting known or suspected violations of applicable rules, and cooperating fully with all health system’s investigations and proceedings. EDUCATION Required: Graduation from an accredited college or university. Graduation from an accredited medical school. Successful completion of an accredited Internal Medicine residency program. Successful completion of additional educational programs necessary to enable Physician to acquire and maintain privileges to perform the range of services contemplated. This includes board eligibility/certification in the specialty being practiced. Attend mandatory educational offerings as described in SCHS policies/procedures and/or as required by other regulatory bodies. Complete annual CME as required to maintain license and appropriate Board certification. Complete in a timely manner assigned Computer Based Learning modules. LICENSURE/CERTIFICATION/REGISTRATION Required: Current Oregon license as MD / DO Federal DEA number registered in Oregon. Must be board certified or eligible in Physician’s chosen specialty. To be board certified or qualified to sit for the boards in primary area of practice at the Hospital. Those applicants who are not board certified at the time of application but who have completed their residency or fellowship training are required to become board certified within seven years of residency or fellowship training. Unrestricted Federal DEA number. Unrestricted ability to participate in Medicare / Medicaid, or other federal or state governmental health care programs. Valid driver’s license and the ability to meet SCHS driving requirements. Current BLS certification required. Additional certifications may be required based on privileges granted to provider by Medical Staff Services at SCHS Inpatient locations. EXPERIENCE Required: Experience using EMR (Electronic Medical Records) software. Advanced written and verbal communication skills to effectively interact with a diverse population and professionally represent St. Charles Health System. Experience working in a team environment and proven ability to collaborate, manage facilitation and consensus building among health care professionals and agencies to achieve expected results. Preferred: 2 years of post-residency experience in Internal Medicine Experience in both inpatient and outpatient practice environments Epic EMR software experience. PERSONAL PROTECTIVE EQUIPMENT Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely. GENERAL Must have excellent communication skills and ability to interact with a diverse population and professionally represent SCHS. Strong teamwork and collaborative skills. Ability to multi-task and work independently. Attention to detail. Excellent organizational skills, written and oral communication and customer service skills, particularly in dealing with stressful personal interactions. Intermediate to advanced proficiency with Electronic Medical Record systems. Intermediate in Microsoft applications (Word, Excel). PHYSICAL REQUIREMENTS: Continually (75% or more): Standing and walking, keyboard operation, use of clear and audible speaking voice and the ability to hear normal speech level. Frequently (50%): Sitting, lifting/carrying/pushing or pulling 1-25 pounds. The use and operation of a motor vehicle for Home Health and Wound Caregivers.
