  1. Job Summary 

The Transportation Aide will be responsible for providing safe and efficient transportation of residents to medical appointments and throughout the facility in compliance with current applicable federal, state, and local standards, guidelines, and established policies and procedures. Transports residents to appointments and performs direct resident care duties under the supervision of licensed nursing personnel, as needed.

Residents Rights 

  • Knows and respects patients’ rights. 

  • Ensures protected health information is kept confidential. Agrees not to disclose resident’s protected health information and promptly reports suspected or known violations of such disclosure to the Executive Director. 

  • Ensures that all care is provided in privacy with respect and dignity. 

  • Reports complaints made by patients to supervisor. 

  • Reports allegations of patient abuse, neglect and/or misappropriation of patient property. 

Safety and Sanitation 

  • Follows established safety policies and procedures. 

  • Observes safety needs of patients as indicated in care plan. 

  • Wears and/or uses safety equipment and supplies when indicated and properly trained to use. 

  • Follows driver safety protocols including defensive driving, vehicle safety walk-arounds, lift safety, seatbelt/restraint safety, restraints, and latch/lock safety. 

  • Understands vehicle accident procedures. 

  • Always demonstrates text-free driving. 

  • Checks transportation equipment daily. Takes unsafe/broken equipment out of use and reports to Plant Operations immediately upon discovery. 

  • Cleans vehicle and transport equipment after use and observes all infection control practices. 

  • Demonstrates job-specific knowledge of fire and disaster preparedness during drills or actual situations. 

Staff Development 

  • Attends and participates in scheduled in-service training, educational classes, and meetings, including safety committee. 

  • Attends and participates in orientation and training as directed. 

  • Assist in the orientation and training of other staff, including back-up drivers. 

  • Completes assigned Relias training. Completes driver training, observations, and skills checks at orientation and annually. 

Transportation and Care Responsibilities 

  • Maintains or assists in maintaining written schedule of appointments. Confirms doctors’ appointments for residents. Updates schedule with missed appointments. Ensures appointments are rescheduled. 

  • Assist in planning and in providing actual transportation to community events. 

  • Notifies family of arrangements regarding appointments. 

  • Arranges appointment times to allow flexibility to remain with patient if necessary. 

  • Delivers and picks-up equipment. 

  • Delivers and picks-up medical documents from physician offices. 

  • Transports laboratory specimens to the hospital or lab. 

  • Runs errands for the facility. 

  • Keeps the company vehicle fueled and maintained. 

  • Conducts daily, weekly, monthly, and annual audits of vehicles per the safety program. 

  • Keeps Plant Operations aware of routine service or need for repairs to vehicle. 

  • Loads and unloads patients from vehicle. 

  • Accompanies patients to medical appointments and stays with patient as needed. 

  • Ensures residents have been provided food or have food available when necessary. 

  • Delivers and returns appropriate documentation with each patient appointment. 

  • Reports and unusual occurrences or incidents involving patient to the charge nurse. 

  • Maintains patient confidentiality regarding any medical information. 

  • Assists residents with resident care including bathing, grooming, hygiene, and placement of adaptive equipment. 

  • Handles linens according to infection control guidelines. 

  • Assists residents with bowel and bladder functions. 

  • Assists residents in preparation for and transporting to appointments, activities, and social programs. 

  • Assists with lifting, transferring, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc., in keeping with specific resident safety needs. 

  • Assists residents in ambulation with or without self-help devices as instructed. 

  • Accurately measures and records heights and weights. 

  • Accurately measures and records temperatures, pulses, respirations, and blood pressures (TPRs and B/P) as instructed. 

  • Ensures that residents’ personal care needs are being met in accordance with residents’ wishes. 

  • Reports all changes in residents’ condition or behavior to supervisor as soon as practical. 

  • Maintains cleanliness of facility vehicle. 

  • Documents mileage for all trips. Maintains Non-emergency Medical Transport log. 

  • Reports all receipts for all vehicle maintenance according to facility policy. 

  • Maintains positive, professional interaction with residents and staff. 

  • Assists with other assigned duties when not transporting. 

Documentation 

  • Accurately documents information in the clinical record as required by the patient’s condition. 

  • Accurately maintains all vehicle and driving logs. 

Administrative Responsibilities 

  • Reports observations concerning equipment defects and malfunctioning to appropriate personnel. 

  • Completes all required documentation for Safety committee meetings. 

  • Maintains confidentiality of necessary information. 

  • Thinks and acts calmly and logically to meet unusual occurrences of the job without being thrown off stride. 

  • Performs any miscellaneous work assignments as may be required. 

  1. Education/Qualifications 

  • Successful completion of State Approved Nurse Aide Training and Competency Evaluation Program; Name must appear on State Registry as Nurse Aide in good standing; Ability to communicate with residents and nursing staff; Must have the ability to read and write. 

  • Must have a current/active CPR certification or obtain within 6 months of hire. 

  • Persons who have been found guilty of abuse, neglect, exploitation, or mistreatment or misappropriation of property by a court of law; or who has a finding in the State nurse aide registry concerning abuse, neglect, exploitation, mistreatment of residents or misappropriation of resident property, or has had a disciplinary action in effect taken against his/her professional license are not eligible for employment in this position. 

  • Must have MVR review free from any disqualifications and meet acceptable driver criteria. 

  • Must be at least 18 years old. 

  • Must have a valid, unencumbered driver’s license in the state in which facility is located. Licenses from other states will not be allowed. 

  • Approachable, flexible, and adaptable to change. 

  • Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity. 

  • Function independently, and have flexibility, personal integrity, and the ability to work effectively with employees and vendors. 

  1. Experience 

  • One (1) year of experience as Nurse Aide. 

  • Prior experience in transportation or driving preferred. On-the-job training provided. 

  1. Physical Demands 

  • Lifting 50 pounds maximum with frequent lifting, moving, and/or carrying of objects weighing up to 50 pounds; Exerting considerable force frequently, and/or intermediate force constantly to move objects and residents. Reaching, handling, fingering, talking, hearing, visual near acuity, visual acuity, bending, stooping, and walking. 

  • Sitting for extensive periods of time. This position requires frequent driving and may be in a vehicle for extended periods of time. Repetitive gripping throughout the day. 

  1. Travel 

  • Extensive; Must be able to work beyond normal hours and on weekends and holidays when required.

  • Must be able to work flexible hours as needed. 

  1. Working Environment/Conditions 

  • Considerable exposure to disagreeable conditions including odors, infection, illness, and physical contact from disruptive residents. 

  • Exposure to road noise and potential safety hazards on the road. 

  • Tasks assigned to this position may involve potential and/or direct exposure to blood, body fluids, infectious diseases, air contaminants, and hazardous chemicals.
