SSTAR is a Federally Qualified Health Center (FQHC) which affords our employees eligibility to apply for one of our three Loan Repayment Programs.

The Digital Navigator is a specialized role developed to address patient’s and communities’ digital access barriers and telehealth opportunities. The Navigator will be trained in a number of resources focused on educating patients how to initiate and participate in a telehealth visit and, if needed, how to use remote patient monitoring equipment. The Navigator will assist patients with enrolling and utilizing the Patient Portal.

Compensation: $22-23/hour

Duties & Responsibilities

  • Provides digital literacy education to patients and families with the goal of increasing confidence and moving patients to video telehealth visits.
  • Perform Digital Equity Screening with patients to assess needs.
  • Utilizers EMR tools and provider relationships to identify patients experiencing core barriers to digital equity.
  • Offer portal enrollment and support to patients, understanding that provider communication outside of visit is vital to care.
  • Works to reduce barriers that may complicate video telehealth visits.
  • In partnership with the supervisor of Patient Benefits and Director of Population Health, develop and execute workflows aimed at bringing qualifying appointments to telehealth,
  • Develops communications, guides and other materials aimed at properly preparing patients for video telehealth visits.
  • Screen patients for social determinants of health and refer to community services.
  • May have to travel among three Health Center locations to meet with patients..
  • Other duties as assigned.

Experience and Skills:

Education & Experience

  • Experience in a community health center setting and/or related experience in health/ human services, social services agency preferred.
  • Associates or BA in health or human services preferred.
  • Demonstrated strong communication skills with individuals and/or groups.
  • Experience working with patients with chronic behavioral health needs a plus.
  • Strong analytical, problem solving and planning skills.
  • Strong computer skills and ability to quickly adapt to using an electronic health record and Microsoft Office Applications.
  • Ability to successfully prioritize multiple projects on an ongoing basis.
  • Self-motivated and self-directed to work independently as well as ability to engage with various teams across the organization.
  • Maintains strict adherence to confidentiality policies.
  • Bilingual is a plus.
