CARES Program:

Connection and Resources Equity Services, CARES, is a program that was created to address the social determinants of health by offering the opportunity to connect families to community resources while offering supportive follow up and building trust within their medical home.

Danis Pediatrics:

  • Outpatient pediatric primary care services
  • Two locations – Cardinal Glennon and Tower Grove
  • Provider care for patients newborn to age 18
  • Teaching clinic – providers, nurse practitioners, physician assistants, social workers, medical students
  • Large percentage of families in poverty

Care Coordinator:

  • Care coordinators are available at each location to connect our families with resources to meet their needs. 
  • Meet with families in clinic and stay connected via phone or email to ensure that they are making connections and ensuring that they have access to the resources available to them.
  • There is not a time limit to this support – focus on meeting the families where they are at and offering guidance and supportive care

Needs Assessment Tools Utilized by the CARES Team:

  • Family Well Being Form (FWBQ) – offered to families at every well child visit from newborn to 18 years. 
  • Edinburgh (EPDS) – a depression screening that is offered to post-partum mothers 6 times starting when the child is 7 days old through 6 months old.
  • Community Partnerships: Danis CARES Team works with organizations in our community to build relationships and access to resources to improve the lives of our patients and their families.
  • Medical Students – 1st year of medical school. The CARES program rotation is a chosen elective. They assist with updating resources and community connections while shadowing the team in clinic.

Job Summary:

Responsible for detailed preview of charts for high complexity patients and patients in the population health programs. Works with providers across the organization to ensure accurate coding for diagnosis. Assists in providing cost-effective healthcare services through the treatment process. Serves as a liaison between patients, physicians and all other participants of the health care team.

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

  • Reviews charts to identify potential diagnosis codes so that risk factors can be properly addressed by physician during a risk assessment visit.
  • Conducts audits including research diagnosis and health maintenance treatment.
  • Schedules appropriate transition of care and assists licensed personnel to enhance comprehensive patient care and reduce gaps.
  • Identifies ongoing health maintenance needs and facilitates next steps for those needs.
  • Performs other duties as assigned.

EDUCATION

  • High school diploma or equivalent

EXPERIENCE

  • One year experience

PHYSICAL REQUIREMENTS

  • Constant standing and walking.
  • Frequent use of hearing and speech to share information through oral communication.
  • Frequent use of hearing to distinguish body sounds and/or hear alarms, malfunctioning machinery, etc.
  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
  • Frequent reaching, gripping, and keyboard use/data entry.
  • Frequent use of vision for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
  • Occasional bending, stooping, crawling, kneeling, sitting, squatting and repetitive foot/leg and hand/arm movements.
  • Occasional use of vision to judge distances and spatial relationships.
  • Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
  • Occasional lifting/moving patients.
  • Occasional use of smell to detect/recognize odors.
  • Occasional driving.
