Position Summary:

As a key member of our healthcare team, the Social Director is responsible for planning, organizing, implementing, evaluating, and directing the Social Services Department. The role ensures compliance with federal, state, and local standards, as well as facility policies, to meet the emotional and social needs of our patients/residents on an individual basis. In adherence to regulatory requirements, this position requires a full-time social worker with a minimum of a bachelor's degree in Social Work or a related human services field.

Key Responsibilities:

  • Participate in the planning, development, organization, implementation, and evaluation of social service programs within the facility.
  • Work closely with administration, medical and nursing staff, and other related departments to plan and deliver effective social services.
  • Develop and maintain positive relationships with intra-department personnel, other facility departments, and external community health, welfare, and social agencies.
  • Stay informed about current federal and state regulations, making recommendations for policy and procedure changes as needed.
  • Participate in patient/resident assessments, contribute to the development and implementation of social care plans, and engage in discharge planning.
  • Conduct private interviews with patients/residents and their families, ensuring open communication about personal and property rights.
  • Assist in arranging transportation for patients/residents to other facilities when necessary. Refer patients/residents/families to appropriate social service agencies when the facility is not the provider of needed services.
  • Provide consultation to facility staff, community agencies, and other stakeholders to support patient/resident needs and resolve issues through social service programs.
  • Maintain timely and accurate documentation of social services notes as necessary, adhering to facility policies.
  • Uphold patient/resident confidentiality, ensuring the protection of their rights.
  • Review and address complaints and grievances, reporting necessary information to the department manager.
  • Support medically-related emotional needs, assisting patients/residents/families in coping with anxieties, stress, and other emotional challenges related to illness and admission to the facility.
  • Interpret social, psychological, and emotional needs of patients/residents/families, communicating these needs to the medical staff and care team.
  • Assist in obtaining resources from community social, health, and welfare agencies to meet the diverse needs of patients/residents.

Qualifications:

  •  Social Service Certificate, 36-Hour Social Service Designee Certificate, preferred. 
  • One year of supervised social service experience in a healthcare setting working directly with geriatric individuals. 
  • Minimum of 2 years’ experience in a Skilled Nursing Facility preferred.
