Title: Social Worker
Job Description:
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will be committed to improving the lives of individuals and families facing various challenges by providing support, advocacy, and resources. As a Social Worker, you will work closely with clients to assess their needs, develop treatment plans, and connect them with appropriate services. Your role will involve collaborating with other professionals and community organizations to ensure comprehensive care and support for clients. This position requires strong communication, problem-solving, and interpersonal skills, as well as a deep understanding of social justice principles.
Responsibilities:
Conduct assessments to identify clients' strengths, needs, and goals
Develop and implement individualized treatment plans in collaboration with clients
Provide counseling and support services to individuals, families, and groups
Advocate for clients' rights and access to resources within the community
Coordinate referrals to external agencies and services, such as healthcare providers, housing assistance programs, and support groups
Maintain accurate and up-to-date records of client interactions and progress
Collaborate with interdisciplinary teams, including healthcare professionals, educators, and law enforcement, to ensure holistic care
Stay informed about relevant laws, policies, and resources affecting clients' well-being
Participate in ongoing training and professional development activities
Qualifications:
Required: LCSW Cert
Required BLS.
Must have Acute Care Hospital experience.
Job Type: Contract
Pay: $45.00 - $50.00 per hour
Expected hours: 40 per week
