Job DescriptionSpring Creek Rehab and Nursing is seeking motivated and compassionate Licensed Practical Nurses to join our team!We have Full time and Part-Time shifts available.Day shift 7am-3pm -part timeEvening Shift 3pm - 11pm - full time and part timeNight Shift 11pm-7am -full time and part timeLPN with trach and vent exp - 12 hour day shift and 12 hour night shiftThe LPN is responsible for the following:Monitoring residents, their responses to treatment care plans.Supervising and directing.Document necessary information regarding nursing care, care plans and observation of residents overall conditions.Perform various duties to provide quality nursing care to residents.Analyzes information for appropriate interventions to prevent decline.Record signs of change in condition.Notify appropriate staff per facility protocol to evaluate weight loss, decline in ambulation, development of skin breakdown, etc.Administers medications and treatments to residents; monitor administration of intravenous fluids; places orders for medication.Maintain resident clinical files; keep charts updated; document appropriately.Make rounds to see each resident daily.Job RequirementsAt least one year of experience in a Long Term Care environment is highly preferred.Must be a graduate of an approved practical nursing program.Must possess and maintain a current state PA license as a Licensed Practical Nurse (LPN)Must be able to move, lift and position patients.Because we know you are our strongest asset, we offer an attractive benefits package for full-time including medical/dental insurance, Updated wage scale now in effect!
