Description

Qualifications: Masters Degree in Speech Language Pathology from and accredited program. Certificate of Clinical Competency or eligibility for Clinical Fellowship Year. Assumes responsibility for ongoing continuing education and professional development. Current state licensure or license eligible.
As a Speech Therapist in a Grace Management Services facility you must be able to perform the following essential functions as demonstrated by the ability to:

Essential Job Functions:
Provides speech therapy services to residents and initiates referrals when appropriate and in accordance with the Therapy Department Procedures.
Conducts screening of residents at regular intervals to determine need for intervention/treatment.
Evaluates residents to obtain data necessary for treatment planning and implementation. Conducts specialized evaluations as indicated.
Interprets and communicates evaluation findings to residents, families and team members.
Establishes individual resident schedules in conjunction with other team members.
Develops treatment plans by identifying a target list, including short and long-term goals and methods to achieve identified goals.
Coordinates treatment plan with resident, family and other team members.
Implements and conducts treatment as outlined in treatment plan.
Monitors residents response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders.
Develops appropriate home or programming within the community to maintain and enhance the performance of the resident in their own environment.
Documents results of residents assessment, treatment, follow-up and termination of services.
Adapts treatment to meet the individual needs of the resident.
Provides, recommends, fabricates adaptive devices, or other equipment. Trains resident, family and appropriate staff in the use or application of orthotics and prosthetics.
Instructs resident, family and staff in appropriate speech-language procedures or home program to allow carry over of therapeutic remediation and goals in daily activities.
Participates in discharge planning.
Supervises Rehabilitation Aides, students and volunteers.
Reviews the quality and appropriateness of the total services delivered and individual occupational therapy programs for effectiveness and efficiency, using pre-determined criteria.
Participates in resident, family and staff education.
Confers formally and informally with other team members in coordinating the total rehabilitation program of the resident.
Adheres to Speech Therapy Department procedures regarding documentation of speech therapy services.
Provides in-service education to the members of the residents care team and education to the community.
Provides treatment in accordance with established standards of practice, department procedures and productivity standards.

Consistently demonstrates sound judgment in the evaluation, planning implementation and follow-up of resident therapy
programs.
Reports to work on time and coordinates schedule to achieve maximum productivity and efficiency during assigned shift.
Assists with and participates in Quality Assurance Program as required.
Maintains a current knowledge of law and regulations regarding rehabilitative services and consults with supervisor to
ensure that all employment actions are in compliance with applicable regulations.
Attends and participates in scheduled training, educational classes and meetings required or requested for position.
Ensures a safe environment and utilizes tools and equipment in a safe manner complying with all infection control
universal precautions and OSHA standards for the healthcare professional.
Reports all hazardous conditions including incidents, injuries and equipment to Supervisor/appropriate personnel
immediately.
Adheres to high standards of cleanliness, grooming, hygiene and dress code.
Adheres to all policies of company, including job description, certification requirements, mission statement, HIPAA,
confidentiality standards and resident rights.
Adapts to changes in responsibilities in a positive manner, is receptive to new ways of completing tasks and is flexible in
carrying out assignments.
Communicates and interacts professionally and effectively with residents and coworkers throughout the community
ensuring a cooperative environment while maintaining self-control.
Communicates and interacts professionally and respectfully with family members in attending to and meeting their
requests.
Perform all other duties as assigned and as required to effectively discharge the responsibilities of the position and are
in the best interests of the organization.

Sit or stand for extended periods of time
Reach above shoulder heights, below waist or lift as required to complete the job responsibilities
Proper lifting techniques, ability to read computer screens and the ability to follow written and verbal instructions are required
Must be able to lift 20 pounds. Raising objects from lower to a higher position or moving objects horizontally from
position to position. This requirement occurs to a considerable degree and requires the substantial use of the upper
extremities and back muscles.
Must be able to balance, climb, crouch, grasp, kneel, pull, push, reach, stand and stoop adequately to perform the
essential functions of the job.
Must be able to hear, smell, feel and speak adequately to perform the essential functions of the job.
Must be able to read and write the English language.
Must be able to work inside and outside with exposure to outdoor weather conditions
