JOB PURPOSE:
The Hope Squad Liaison willserve as a liaison between local ISD campuses and local program advisor trainerto reduce youth suicide through education, training and peer intervention. Theschool based liaison works in collaboration with and provides training toprogram advisors and other school personnel as part of their job duties. Utilize your "people skills"and your social science knowledge in combination with extensive on the jobtraining and clinical supervision to provide an array of crisis, carecoordination, case management, assessment, recovery planning and skillstraining services to children, adolescents, adults or families. May participate in initial and ongoingassessments utilizing a "trauma informed care perspective" toidentify prior experiences, needs, risks, barriers, goals, objectives,abilities, strengths, supports and preferences, and help facilitate theintegration of this information into person and family centered, recovery,crisis, safety or referral plans. Basedon these plans, may provide face to face case management and/or carecoordination services including making and following up on referrals, identifyingand overcoming barriers to accessing needed services and supports, and engagingor coordinating informal and professional supports in the recoveryprocess. Utilizing approved curriculum,may provide psychosocial skills training for children, adolescents, adults andparents. Provides intervention servicesas determined by risk assessment and consumer needs. Uses all informationobtained to guide changes to the recovery plan and refers to the mostappropriate services as needed. Collaborates with other professionals on thetreatment team and other healthcare, social service, educational and criminaljustice organizations. May conductcommunity outreach activities. Completesall documentation necessary to document services performed.
EDUCATION AND EXPERIENCE REQUIRED:A bachelor's degree from an accreditedcollege or university with a major in social, behavioral or human services oris a registered nurse. (Social,behavioral, and human services are: psychology; social work; medicine; nursing; rehabilitation; counseling;sociology; human development; physician's assistant; gerontology; educationpsychology; special education; early childhood intervention; or early childhoodeducation.)
DEGREE AND/OR CERTIFICATION REQUIRED (IFAPPLICABLE): BA/BS or RN
