DUTIES AND RESPONSIBILITIES
· Offer structured, one-to-one mentoring support services to youth to address daily living, social, and communication needs
· Educate, coach, and support youths in socialization, problem-solving, proper behaviors, resolving conflicts, and creating appropriate relationships with other youths, as well as adults, in different social settings
· Observe interactions in different settings and discuss strategies for handling specific interactions
· Generate a support plan to include client, associated service provider(s), and team member input, and distinctly identify areas of concern, strategies, and service objectives
· Complete progress reports in a timely manner, as communicated by supervisor
· Participate in Family Assessment and Planning Team, and other integrative, analytical, or provisional meetings
· Work in partnership with other case involved service providers and professionals (ie. attorneys, school staff, social workers, and probation officers)
· Connect clients with other programs or services, such as healthcare services, employment resources, recreational activities, transportation resources, financial resources, transportation resources, and others
· Attend court hearings when families are court-involved and presents testimony and treatment recommendations as requested by attorneys, referral sources, the court, or clients
· Collaborate with Supervisor to strengthen efficacy of support plan
· Develop Discharge Summary and Strategy
· Attend monthly trainings as provided by agency
· Document and convey progress toward identified goals regularly to the youth's service provider(s)
