GENERAL STATEMENT OF DUTIES
Teaches Direct Support staff how to support people with disabilities, both by example and as outlined in each individual’s Person Centered Plan. Creates positive and supportive teamwork atmosphere within the home, where all members contribute and are valued. Provides leadership to the team and to individual staff, providing support and direction to staff. Manages and promotes the smooth operation of the home, including maintenance, safe environment, budget, staff-related issues and vehicle-related issues. Identifies opportunities for improvement and actively participates in their implementation. Meets or exceeds program quality standards and identifies and acts on areas for improvement. Supports culturally competent recovery-based practices and person centered planning as a shared decision making process.
RESPONSIBILITIES
Teach Direct Support Staff how to support people with disabilities, including specific needs of individuals.
Coordinates the specific supplies and equipment needs of individuals (medical, adaptive equipment).
Participates in individuals’ Person Centered Planning. Leads the staff in implementing the plans and recommending necessary changes to the plans.
Provides safekeeping of individual’s funds, including accurate accounting of all expenditures and monies received.
Assist in providing direct support to individuals in order to role model for staff and as needed.
Ensures that individuals receive all assistance for which they are eligible (i.e. food stamps, Medicaid, Medicare, Home Health, SSI, etc.)
In all interactions, communicates with others in a clear, effective and respectful manner.
Leads the team through daily interactions, staff meetings, goal setting and promotion of the agency mission.
Supports individual staff to improve their skills. This involves providing scheduling training, role modeling, developing plans for improvement and providing opportunities for career growth/advancement.
Provides and schedules training for all staff to ensure that all staff are fully competent in all areas of responsibility.
Creates a cohesive home/team environment that is supportive and recognizes the
contributions of all members.
Involves all staff in the daily operations of the home, delegates responsibilities according to staff strengths.
Recruits and hires staff for positions within the program.
Follows agency policies and applicable laws regarding all personnel related issues.
Communicates procedures and policies to staff. Conducts work within these guides.
Ensures that documentation of supports is provided (medications, health care, activities, skill building programs, financial, etc.).
Operates program within budget parameters. Shares budget information and reports with staff.
Arranges maintenance of home and vehicle, ensuring they are safe, clean and inviting.
Provides for smooth operation of home through effective scheduling, routines and communication.
Completes all required reports timely.
Completes all other duties as assigned by Administration.
Participates in development of Continuous Quality Improvement plans. Makes the plans part of how work is done every day. Communicates plans to staff and others, promoting their goals.
Monitor program outcomes, to see that program is meeting/exceeding guidelines.
Manages program in manner that meets or exceeds expectation of stakeholders, accreditation and regulatory bodies (such as CARF, CMH, AFC Licensing).
