Patient Care Advocate
03 months
Location : Saginaw / Genesee counties.
Can travel up to 2.5 hours.
Monday - Friday 8-4 : 30 with 1 / 2hr lunch or 8-5 with 1hr lunch
Candidate must live within the Counties identified above and be willing to travel up to 2.5 hours from home address.
Required Testing :
Driver's license required.
Clinical is highly preferred.
Position Purpose :
- Works with members and providers to close care gaps, ensure barriers to care are removed, and improve the overall member and provider experience through outreach and face-to-face interaction with members and providers at large IPA and / or group practices.
- Meets 80% of visit requirements depending on the assignment could be monthly, bimonthly, or quarterly.
- Be on time, be available and responsive to internal and external communication, and increase individual and overall measure rates.
- Accurate documentation of workload.
- Introductions, checking access, sharing day to day expectations, assigning to a buddy.
- Highly encouraged to have clinical experience, could be provider office support, working with both members and providers.
- CNA, LPN, office manager in provider setting, front of the house member facing support (receptionist with 2 years experience calling, scheduling, and responding to member needs.
Internal / External Groups with which the Candidate will interface :
Members, Providers, multiple health plans and corporate departments
Required Skills / Experience :
Proficient in Outlook, excel, and PowerPoint (must be able to use Outlook for communication to providers, track visits on the calendar, utilize Excel for tracking and documenting data, and use PowerPoint to create presentations for education.
Project Management / Lead
Education Requirement :
Bachelor's Degree in Healthcare, Public Health, Nursing, psych, Psychologyial Work, Health Administration, or related health field or equivalent work experience
