The Clinical Care Coordinator Role:
The Specialized Residential Services (SRS) Client Care Coordinator will collaborate/coordinate and oversee person-centered plans, behavioral assessments and support plans. This person will be responsible for communicating with agency and community partners regarding psychological assessments and testing, as well as executing all operational logistics related to plan implementation. This role will provide training and support of person-centered and behavior management plans for direct care staff, case managers and persons served. This vital role will serve as an on-site expert in the interest of enriching the client care experience across the board. The Client Care Coordinator will work in partnership with our Client Care Advocate to ensure a well-rounded approach to coordinating daily living at all SRS facilities as it relates to behavior management and general quality of living.
Responsibilities:
1. Attend person-centered planning (PCP) meetings.
2. Adhere to all Behavioral Treatment Committee (BTC) and PCP, and agency and contractual requirements for behavioral programming and management.
4. Responsible for environmental & behavioral analysis for persons served.
5. Responsible for monitoring of person-centered and behavior treatment plan goals.
7. Direct staff training in the implementation and documentation of behavior programs.
8. Act as liaison in collaborating with referral agencies, and respective medical and psychiatric providers where appropriate.
