JOB DESCRIPTION
Title: Driver Division: HOPICS/ Interim Housing
FLSA: Non-Exempt, Full-time Supervisor: Interim Housing Program Manager
Pay Range or Rate: $20 to $23 per hour Revised: 02/12/2024
Summary
Under the Lemon House Program's direction, the Driver will assist with delivering direct services via safe transportation to and from program activities. Transport families and/or participants between housing facilities, social service agencies, potential housing units, and program sites.
Essential Functions
• The tasks, duties, and responsibilities of the job include, but are not limited to, the following:
• Transport all scheduled appointments.
• Maintain records of mileage and destination of all client transport.
• Maintain documentation related to gas card usage.
• Communicate and coordinate with program staff to schedule and map out client transportation needs.
• Follow all safety procedures and protocols.
• Transport Inter-office mail.
• Transport materials between program sites.
• Maintain records on the maintenance of the transport vehicle, such as tune-ups, brakes, oil changes, and washes; always keep all properly updated documentation in the vehicle.
• Maintain a record of car washes and cleanliness of vans.
• Transport clients between facilities, social service agencies, potential housing units, and program sites.
• Operate company vans and larger vehicles in accordance with federal and state diving guidelines and safety codes.
• Adhered to division policies and confidentiality, and treated clients, co-workers, and partners with respect and dignity.
• If the participant is enrolled in any programs and transportation service was provided, a case note must be entered within 24 hours.
• Represent the Agency in a professional manner at meetings and community events.
• Case Notes are to be entered within 24 hours in the DAP (Data, Assessment, Plan) form. Additionally, case notes will be detailed and grammatically correct.
• Submit the End of Shift Transportation Report before your work shift ends daily.
• Check and Accept transportation requests daily and periodically throughout the day.
• Add/ Update all transportation requests to Google Calendar
• Attending weekly case conference meetings as needed.
• Provide the Supervisor/Program Manager with all required outcomes as requested.
• Maintain appropriate boundaries and adhere to SSG’s Code of Ethics and HOPICS’ Core Values.
• Regular attendance is required.
• Other duties as needed.
Secondary Functions
Perform other duties as assigned by the Supervisor, Program Manager, Division Director, and Deputy Director. Maintain files/records on client services in compliance with HIPAA, 42 CFR Part 2 and other funding requirements for audit purposes. Overtime, holiday, or weekend work may be required.
Allow time for completion of required reports and documentation. Attending mandatory Agency and Division meetings and trainings.
Minimum Qualifications - Knowledge, Skills and Abilities Required
Submission of an H-6 printout from the California Department of Motor Vehicles. A Valid California Driver's License (or higher) and Automobile Insurance are required. Ability to communicate effectively with minimal supervision and as a team member. Working knowledge of Microsoft Word, Excel, and other database programs. Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds. Ability to communicate effectively, both written and orally. Verification of Employment Eligibility and Background Clearance. TB test required (Not more than (3) months prior to or (7) days after the Date of Hire, and renewed annually thereafter), CPR and First Aid Certification required within 30 days of employment with the company, and a valid Driver’s License and auto insurance required. Reliable transportation is required.

Supervisory Responsibilities:
This position does not have any supervisory responsibilities.
Environmental Conditions (Working Conditions)
This position is responsible for working in “client-friendly” environments and is required to visit other shelters and homeless access centers as part of their duties. Local automobile travel is required. There is some responsibility to work in noisy environments where children and adults are present.
Physical Requirements
The Driver typically spends time sitting, standing, typing, writing, walking, driving, carrying (max. 50 lbs.), listening, and speaking.
Mental Requirements
This position will require the individual to handle any/all of the following: constant distractions, interruptions, uncontrollable changes in priorities/work schedules, and be able to process information, think, and conceptualize.
