This provider will be delivering primary care services to patients of all ages in an outpatient setting while directly overseeing all award-funded staff. They will be responsible for monitoring the clinic's progress on the goals, objectives and activities detailed in the work plan ensuring that the award activities are carried out. Provides comprehensive health care to patients in collaboration with physicians and other medical staff including medical evaluation, counseling, and referrals. Assesses the physical & psychosocial status of patients by means of interview, health history, and physical examination and diagnostic tests. Interprets the data, develops, and implements therapeutic plans, and follows through on a continuum of patient care across the health-illness continuum and life span.
