Provides nursing services to members primarily assigned to a specific clinical location. Services include engagement, assessment, triage, treatment planning, medication administration, health and medication education, physical healthcare advocacy, and supportive care. Has an assigned caseload and is available as needed to any member assigned to their clinic. Participates in utilization management and quality improvement activities as required. Provides after hours coverage and support to all persons being served on the ACT team.

Our Assertive Community Treatment (ACT) teams use a multidisciplinary approach to provide comprehensive, community-based treatment to our members with persistent and severe mental illnesses and/or substance use disorders. Team members provide a full array of integrated, intensive supports and services around the clock. This evidence-based practice helps improve outcomes for people who are most at-risk of homelessness, psychiatric hospitalization, and institutional recidivism.
