Westmoreland Enhanced Personal Care Home provides meals, supervision, assistance with personal care tasks to those who are unable to care for themselves but do not need skilled care. Services vary and are based on the individual needs of each resident. Personal Care Homes are inspected and licensed by the Pennsylvania Department of Human Services. The program is designed to provide a home like environment that provides a level of security to the residents, while they receive a complement of services designed to meet their individual needs.

Seeking Licensed Practical Nurses who are graduates of an accredited school of practical nursing and are licensed by the Commonwealth of Pennsylvania. In your role as an LPN, you will provide patient care in the personal care home under the direction of a Registered Nurse and Supervisor, within the parameters established by State and Federal Regulatory Agencies and Policies. In addition, you would be responsible for reporting changes in the patient’s condition as well as monitor their progress and environment on an ongoing basis. You must be organized and able to multi-task and complete all required documentation in an accurate and timely manner. Our team is made up of dedicated professionals whose main goal is to provide encouragement and support to all the residents.

• Graduate of an approved Practical Nurse Program
• Current and valid LPN licensure to practice in the Commonwealth of Pennsylvania.
• Previous home health care experience preferred.
• Must successfully obtain Criminal History Clearance from the PA State Police, Child Abuse Clearance from the PA Department of Human Services and FBI Fingerprint Clearance.
• Must not be excluded from participation in Medicare, Medicaid, or any other federal health care program.

• Ability to transport oneself and/or individuals in an insured vehicle, as program needs dictate.
