Salary Range $47,000 to $50,000

The Service Coordinator (Camden County) is responsible for assisting and enabling children and families eligible for New Jersey Early Intervention Services System to receive the rights, procedural safeguards, and services that are authorized by the New Jersey Early Intervention Program. This individual coordinates services and advocates for families to assure linkage to all services provided within the Early Intervention Program and with other community resources in order to maximize their child’s abilities.

Essential Functions:

* The Service Coordinator facilitates and participates in the development, review and evaluation of individualized family service plans with family input. * Coordinates the performance of family assessments and evaluations using approved program criteria, client intake and data collection/documentation tools. * Acts as a point of contact, assisting families in identifying service providers and other services that enable them to participate fully in their natural environment. * Coordinates and monitors the delivery of available services. * Advocates for and informs families of available advocacy services. * Facilitates the timely delivery of available services. * Facilitates the development of a transition plan to school-based services. * Documents all service coordination activities and other reports in a timely, accurate and complete manner. * Maintains client and staff confidentiality. * Provides services in a culturally competent manner. * Demonstrates sensitivity, knowledge and an understanding of the challenges families with a child with disabilities may experience.

Requirements

  • Bachelor’s degree required
  • Must meet the NJEIS Personnel Standards for Service Coordinator as listed on: http://www.state.nj.us/health/fhs/eis/documents/njeis_pra.pdf
  • Basic computer skills (Word, Excel)
  • Ability to communicate clearly both orally and in writing
  • Skills and experience in case management, community outreach, group process and parenting education
  • One year experience working with children age birth to three
  • Good organization skills, problem-solving skills with ability to prioritize work requirements
  • Ability to work well with families and providers
  • Ability to work with diverse populations; demonstration of cultural competence
  • Successful completion of system training including orientation and procedural safeguards
  • Valid driver’s license, current insurance policy and car
  • Bilingual English/Spanish Preferred

Physical Requirements:

  • Must be able to see to read documents and hear well enough to communicate with others
  • Must be able to write, type on a computer, and use a telephone
  • Must be able to operate a motor vehicle
  • Must be able to lift up to 20 pounds
  • Additional requirements to be determined with input from Director and HR

Work Environment:

Telecommutes with daily travel within the assigned county/contiguous counties where traffic, weather, and other environmental conditions may be encountered
