$/hr minimum - $/hr maximum (range increases based on education, experience and certifications)

Responsible for providing and coordinating diabetes education services and assistance to patients, families and caregivers referred for SIH diabetes care services. Will guide navigation of referred patients to appropriate outpatient diabetes education services within the SIH accredited program and/or other diabetes support programs or services.

Responsibilities

Role Specific Responsibilities

  • Assess patient needs throughout navigation, identifying unmet needs with appropriate SIH and community based diabetes related services
  • Assesses and improves departmental performance for diabetes care.
  • Assists the diabetes patient with navigation through diabetes self management education and support services.
  • Acts as a ‘safety net’ support for identified high risk patients transitioning from inpatient to outpatient services through phone or face to face contact.
  • Assists in planning and coordination of care for the identified high risk diabetes patient from hospital to first PCP appointment.
  • Coordinates patient transition and navigation through available SIH and appropriate community resources.
  • Develops and fosters collaborative relationships with hospital departments to facilitate and support quality diabetes care and transition services.
  • Collaborates with program medical director, physicians, advanced diabetes nurse coordinators, and other health care professionals to provide clinical and system oversight for the care of diabetes patients, ensuring the provision of efficient, quality, cost-effective care.
  • Communicates with primary care providers with recommendations for ongoing diabetes self management education and support services.
  • Implements SIH Diabetes program initiatives.
  • Participates in quality improvement initiatives, tracks interventions and outcomes using the Six Sigma Lean Daily Management Process.
  • Collaborates with inpatient advanced diabetes nurses coordinators to identify high risk patients with diabetes that need hospital to community transition services to prevent injury, or re-hospitalization related to adverse medication events and to reinforce diabetes self management principles.
  • Participates in SIH hospital based and community diabetes care activities
  • Provides direction to office support staff for ‘single point of entry’ referrals for diabetes education services.
  • Provides group and individual diabetes education services with appropriate documentation, billing, and monitoring per inpatient and outpatient accredited body requirements.
  • Serves as a secondary resource for SIH hospital staff regarding diabetes care issues.
  • Monitors patient response to care/services provided and communicates concerns to healthcare providers.
  • Completes CDE within one year and maintains continuing education and face to face diabetes education requirements per the CDE credential.
  • Complies with SIH policies and procedures protecting patient rights and the handling of confidential information.
  • Ensure work areas are organized and present a safe, accessible, effective and efficient environment for patients and families.

Qualifications

Education

  • Bachelor of Science in Nursing

Licenses and Certification

  • Valid Illinois Nursing License (RN)
  • BLS
  • CDE within 1 year of hire
