The Case Manager assesses and screens the consumer determining needs and appropriate levels of service, provides outpatient intakes/updates and treatment planning.

Bachelor’s Degree in a Social Work, Counseling, or a related Human Service Field required. At least one year experience in working with consumers with mental illness and/or addiction required.

Job Type: Full-time

Pay: $17.00 - $20.46 per hour
