Job Summary
The primary role of the Valve Clinic Coordinator is coordination of care for TAVR patients and ongoing management of the TAVR program. This includes TAVR program development and growth, patient and staff education, as well as the oversight of multidisciplinary teams. The TAVR Coordinator acts as the liaison between the hospital and clinical practice.
Essential Functions
  • Responsible for assisting patients, families and providers through the continuum of service offerings to resolution of care. Ensures the availability, access and timeliness of services in order to optimize quality of care, coordination and patient experience
  • Monitors and tracks all patients who have been diagnosed with heart valve disease specifically but not limited to:
  • Maintain list of patients for aortic balloon Valvuloplasty, Mitral stenosis, pre TAVR patients, and patients with mitral clip
  • Facilitate tests for patients
  • Prepare patient office records for presentation at valve clinic meeting (cath films, MRA, carotids, TEE, Lab Tests)
  • Maintains outpatient records accurately, completely, timely and confidentially according to policies
  • Conducts pre-procedure patient teaching
  • Post Procedure follow up
  • Gain familiarity with echo criteria and valve protocols and notify physician when questions arise.
  • Identifies patients in need of ongoing diagnostic interventions and related services; and facilitates initial and follow up encounters in multidisciplinary clinic
  • Provides input to the Valve Clinic Advisory Committees in the development of quality programs that include tracking information, care conference presentations, outcome measures and patient satisfaction indices
  • Develops and manages monitoring systems to help meet quality initiatives
  • Responsible for collection and submission of quality data for the STS TAVR Registry and for accurate tracking of all volume and quality requirements to obtain and maintain NCD requirements for payment by CMS
  • Assists in the identification and reporting of quality issues, and in the development of a performance improvement plan for the TAVR Program to assess and improve the quality and efficiency of the program including but not limited to quality assessment, utilization review, risk management, and infection control. Identifies appropriate indicators to monitor and track and in conjunction with marketing, tracks patient data advertising campaigns, and patient referral outcomes
  • Organizes and actively participates in a regular multidisciplinary valve clinic
  • In collaboration with the CVSL Leadership, tracks and ensures appropriate work to drive program growth with the goal of achieving CMS NCD requirements for TAVR payment
  • Works with the Director of Cardiac Outreach to facilitate transfer of potential TAVR patients from the region to the Valve Clinic
  • Demonstrates sensitivity to the needs of patients, families, staff, and physicians. Responds to concerns, questions, and other feedback in timely and thoughtful manner
  • Coordination of information to the hospital for valve procedures
  • Provides and/or facilitates clinical and emotional support, education and resources to patients and their families
  • Works with patients, providers and caregivers to identify possible barriers to care and systematically helps to break down barriers (i.e. Financial, transportation, language, etc.)
  • Facilitates as necessary, timeliness of appointments; works continuously to reduce the intervals between time of abnormal results, diagnosis and treatment. Maintains contact with patients throughout the continuum of care; meets patients at their doctor’s appointments as necessary; addresses questions and concerns
  • Develops and maintains relationships with providers, internal service delivery departments, other caregivers and patients in order to coordinate services within the organization, at outside facilities and within the community
  • With the TAVR Directors and leadership team, assists with the development and implementation of goals and objectives (including staff education) for the Valve clinic
  • Assists with establishment, interpretation, evaluation and revision of procedures, protocols and practices
  • Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution
  • Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and those coworkers, and to report all preventable hazards and unsafe practice immediately to management
  • Functions as a clinical liaison and resource for the TAVR program
  • Exhibits behaviors and actions which create a high level of patient/customer satisfaction, which contributes to positive patient/customer relations and which reflect respect for the patient’s customer’s rights, needs and confidentiality
  • Demonstrates effective communication and human relation skills which promote harmony and teamwork
  • Presents behavior and actions which maintain the credibility, integrity, and positive image of the organization and TAVR program
  • Demonstrates a commitment to the strategic objectives of the TAVR program
  • Functions without direct supervision, utilizing time constructively and organizing assignments for maximum productivity. Maintains acceptable standards in the areas of attendance, professionalism, and establish harmonious working relationships
  • Assumes responsibility for the safety of patient/customers and ensures all patient care is provided in accordance with Hospital and other regulatory requirements to include: JCAHO, OSHA, State, Federal, Infection Control, Patient Safety Goals etc.
  • Exhibits flexibility, adapting readily to changes in the work environment or work schedule
  • Assumes responsibility for professional growth and development by staying current in the field by reading related professional literature and undergoing annual bariatric related training
  • Maintains strict confidentiality of PHI (Personal Health Information) following HIPAA policy and government regulation. Maintains strict confidentiality of employee information
Qualifications
  • Associate's Degree Nursing required
  • Bachelor's Degree Nursing preferred
  • Minimum of 3 years Progressive clinical nursing experience in cardiovascular health experience preferred
  • Experience in TAVR preferred
  • RN - Registered Nurse - State Licensure And/Or Compact State Licensure Upon Hire required
