Hospice | Cape Girardeau, MO | PRN
- Advocate for patients and families
- Work collaboratively with patients, families, hospice team and community agencies involved in providing patient care
- Act as a resource to patients, families, and other health care team members
- Ability to make independent social work decisions
- Complete documentation in an accurate, concise, and timely manner
- Group and team dynamic skills, conduct education, community/support groups/ hospice team
- Develop and maintain knowledge and understanding of community resources
- Provide crisis intervention
- Strong communication and leadership skills
- Ability to be flexible
- Strong clinical skills and critical thinking skills
- Manage caseload, self-scheduling and complete tasks in an organized and efficient manner
- Provide initial and ongoing psychosocial assessments, interventions and develop plan of care
- Coordinate financial assistance for patients based on need. Advocate for financial resources and appropriately distribute resources
- Provide education regarding healthcare decision making and assist with documentation
- Assist with bereavement support to patients families Any other duties as needed to drive to the Vision, fulfill the Mission, and abide by the Values of the organization
- Master's Degree Masters of Social Work required Or
- Bachelor's Degree In Social Work required Or
- Bachelor's Degree In Psychology, sociology, or other related field required
- CPR - Cardiac Pulmonary Resuscitation Upon Hire required
