EDUCATIONAL REQUIREMENTS CHANGED - CASE MANAGER / GERIATRIC SUPPORT SERVICES COORDINATOR

FULL TIME / 35 HOURS PER WEEK

RESPONSIBILITIES: Conducts initial in-home comprehensive assessments and re-assessments of elders. Develop and implement an Initial Individualized Plan of Care (ICP). Contact provider(s) and arrange for in-home social support services based on ICP. Contact Providers monthly by telephone to confirm ICP and initiate changes as needed. Provide access to public benefits and community services as needed. Communicate with Primary care physician (PCP) and Primary Care Team (PCT) on a regular basis. Maintain accurate and current consumer data. Meets with supervisor on a regular basis for supervision and case review.

QUALIFICATIONS: Bachelor’s Degree preferred, but not required. Bachelor’s Degree to be obtained within three years of the start date. Two years professional experience in the care of persons over age 65. Experience with developing and maintaining individualized service plans and with working on a multi-disciplinary team preferred, but not required. Good organizational skills. Strong written and verbal communication skills and the ability to work as a team member is a must. Bilingual candidates including Cantonese and Mandarin speaking candidates are encouraged to apply. Strong computer skills required, including intermediate level experience with Microsoft Word, Outlook, Excel, and database programs. Must have a valid driver’s license and provide own transportation.

SALARY: $49,877
