Responsibilities:
- Provide direct patient care under the supervision of a registered nurse or physician
- Administer medications and treatments as prescribed by healthcare providers
- Assist with physical examinations and perform basic diagnostic procedures
- Collect and process specimens for laboratory testing
- Monitor and document patient vital signs and medical history
- Collaborate with healthcare team members to develop and implement patient care plans
- Provide case management services to patients, including coordinating referrals and follow-up care
- Assist with discharge planning and patient education
- Maintain accurate and up-to-date patient records
Skills:
- Experience in hospice care, providing compassionate end-of-life support to patients and their families
- Proficient in specimen collection, processing, and handling for laboratory testing
- Skilled in performing physical examinations and basic diagnostic evaluations
- Knowledge of basic life support (BLS) techniques and ability to respond to medical emergencies
- Familiarity with case management principles and practices for coordinating patient care services
- Experience working with children in a healthcare setting, providing age-appropriate care
- Understanding of HIPAA regulations and ability to maintain patient confidentiality
- Knowledge of discharge planning processes and ability to assist patients in transitioning from hospital to home or other care facilities
- Proficient in suturing techniques for wound closure, as needed during patient care
- Ability to assist with diagnostic evaluations, such as X-rays or ultrasounds, as directed by healthcare providers
