South King Fire & Rescue is Hiring a Care Coordinator Near Federal Way, WA

JOB
DEFINITION AND SCOPE OF RESPONSIBILITIES:
South King Fire seeks to connect community members in need with appropriate local resources. This position will direct case management and perform intake and referral processes as part of the South King Fire Mobile Integrated Health Program (MIH). Additionally, they will work closely with a Community Health Nurse to respond to residents’ needs for nonemergent medical care, and/or crisis intervention.
WORK WEEK AND HOURS:
Normally works a 40-hour work week consisting of four 10-hour days, Sunday - Saturday. The work schedule may be adjusted to meet the needs of the agency. Work may require evening, weekend, and holiday duty.
SUPERVISION RECEIVED:
Works closely with other members of the MIH team to respond to residents’ needs for non- emergent medical care, social services resources, and/or crisis intervention. The employees of the MIH team report to the EMS Chief or their designee.
ESSENTIAL FUNCTIONS:
  • Demonstrate respect for diversity and a commitment to developing cultural humility. Collaborate with providers and systems to reduce barriers and inequities related to the health and cultural needs of the individual and community/population being served.
  • Assist and advocate for community members with scheduling appointments, arranging transportation, complete applications for programs for which they may be eligible, etc.
  • Respond to patients’ homes with Community Health Nurse or other MIH team member to examine, screen, treat, and coordinate delivery of health and social services to individuals and families in non-emergent and crisis situations due to emotional, behavioral, or mental health problems either referred to the program or identified through the Electronic Health Record (EHR) system.
  • Provide comprehensive care coordination with other providers as part of wrap-around care.
  • Provide follow through and work to bridge the gap in situations where patients historically do not get connected with necessary services.
  • Provide direct clinical services, utilizing evidenced base practices, for acute crisis intervention, acute therapeutic interventions, and case management.
  • Document and maintain electronic and/or printed files and correspondence adhering to HIPPA guidelines. Records shall be maintained for all patient contacts. and interactions in the appropriate record system.
  • Provide care coordination, care management, and patient advocacy.
  • Identify resources and assist with scheduling appointments, transportation, and system navigation for patients with needs such as mental/behavioral health, substance use,
homelessness/housing, financial, and other social service needs.
  • Build and maintain strong relationships with local hospitals, behavioral health and social service organizations, home health agencies, long-term living facilities, Department of Social and Health Services, Adult Protective Services, Aging and Disability Services, Fire Department personnel, and local Law Enforcement agencies.
  • Assist with education of Adult Family Homes as to the appropriate use of the 911 system.
  • Engage in community outreach efforts.
  • Employee may be required to attend program related conferences/training.
  • Other duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES:Knowledge:
  • HIPPA rules and regulations related to the continuation of care.
  • Knowledge of basic medical terminology.
  • Knowledge of social services available within the community, greater King County, and the State of Washington as well as any federally related programs.
Skills and Abilities:
  • Work collaboratively in a team and manage multiple priorities, utilize effective time management skills, and exercise sound administrative and clinical judgment.
  • Communicate professionally with others both verbally, written, and electronically.
  • Ability to exercise independent judgment and initiative in managing multiple projects and assignments.
  • Properly manage, and document patient’s care needs, care provided, and follow-up care as needed.
  • Establishing and maintaining effective working relationships with both external and internal customers.
  • Be able to learn to manage information and generate reports for patients needing follow- up care out of multiple incident reporting systems.
  • Travel to multiple office locations and community members’ homes throughout the SKF district.
  • Ability to work with Microsoft Office 365 Products.
  • Become familiar with the EMS delivery system within the department and King County.
PHYSICAL DEMANDS:
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Employees are frequently required to walk, stand, climb stairs, sit, talk, and hear. Occasionally requires lifting or moving equipment 25lbs or more, with or without assistance. Frequently required to use hands to operate objects, tools, or controls; and reach with hands and arms.
WORK ENVIRONMENT:
Employee will be required to work in multiple settings including offices and fire stations as well as outreach to individuals in person, in homes or who may be living on the streets, in shelters or located in suburban campsites. The work may be performed under physically demanding, stressful, and environmentally diverse conditions, and requires the ability to adjust quickly to changing priorities and demands. Work may require visits to multiple locations including emergency rooms and other medical facilities. Position requires frequent travel in an agency vehicle. While performing the duties of this position, the employee may be exposed to individuals who are irate or hostile. The noise level in the work environment is usually moderate.
EDUCATION AND EXPERIENCE:Education:
  • Bachelor’s degree in social work, Psychology, or a related field.
Experience:
  • Three plus years of experience in case management, care coordination, crisis intervention.
Preferred:
  • Master of Social Work
  • Bilingual
Or:
In place of the above requirement, the incumbent may possess any combination of relevant education and experience which would demonstrate the individual's knowledge, skill, and ability to perform the essential functions listed above.
OTHER REQUIREMENTS:
  • Candidate must pass a criminal background check and drug screening.
  • Valid Driver’s License and driving record free of serious or frequent violations.
  • At least 21 years of age
  • Insurable by department’s insurance carrier
