Social Services Worker 2 (QRTP/CANS Assessor) - Child Protective Services Division / Screening Unit

Probationary Hourly Rate: $20.00
(1-Year Probationary Period)
Mid-Probationary Hourly Rate: $21.08
Post Probationary Hourly Rate: $22.22
Pay Range: $20.00-$28.35

ESSENTIAL FUNCTIONS:
Qualified Residential Treatment Programs (QRTP) Qualified Individual Assessor (“QRTP Assessor”) is responsible for activities related to completion of QRTP level of care assessments, re-assessment, and aftercare planning for children placed in a qualified residential treatment facility. The QRTP Assessor is committed to practicing cultural competence by working respectfully and effectively with people of all cultures, classes, races, genders, ethnic backgrounds, religions, sexual orientations, mental and physical abilities, ages, and appearances in a manner that respects and preserves the dignity of all.
Completes Child and Adolescent Needs & Strengths (CANS) level of care assessment for children placed in a qualified residential treatment as required/identified by Ohio Department of Job & Family Services (ODJFS) guidelines. Tracks children maintained in QRTP and completes a level of care re-assessment as required/identified by ODJFS guidelines. Collaborates with the child, the casework team, family, and service providers, oversees individualized aftercare planning for children exiting QRTP placement. Oversees ongoing compliance with QRTP rules as they are established and/or modified to qualify for reimbursement for placement through Title IV-E funding. Is responsible for monitoring and evaluating the agency's compliance and performance with various state and federal requirements, as well as agency policy.
Facilitates family team meetings, held for the purpose of supporting and educating parents, sharing information, and jointly making decisions with the goal of empowering and strengthening families while keeping children safe and planning for their ongoing stability, care, and protection. The facilitator does not have direct line responsibility of the case but works with the service team to receive referrals, schedule, facilitate, and document family team meetings. Prepares & completes reports, correspondence, case records, etc.; maintains files & records both written and electronic.
Attends and participates in supervision meetings as established by the employee’s chain of command and based on the needs of the employee/workload. Attends and participates in all agency meetings including unit, department, division, and all staff meetings.
Receives & investigates complaints of child abuse & neglect, dependency, or exploitation; provides diagnostic & crisis intervention services; determines need & eligibility for service & intervention; monitors home conditions, as needed on assigned open cases, may testify in court re: facts of case. Must be available to be on-call 24-hour services on a scheduled basis (e.g., on weekends, holidays, after business hours); attends & participates in evening appointments & meetings.
Exhibits regular and predictable attendance.
NON-ESSENTIAL FUNCTIONS:
Any other duties as assigned by Director or designee.
