Employees in this position are expected to be flexible, professional and a team player. Employees will coordinate services for high-risk youth involved with multiple agencies (school, DHR Juvenile Court, etc.) Conduct monthly meetings to address needs and goals. Participate in treatment team meetings with supervisor to identify progress/refer to higher level or lower level of care/transition to Low intensity care services. Coordinate needed services and link youth/family to services available in the community. Maintain billing documentation and active case plan. Must have Bachelor’s degree in a human services related field, ICC/CM trained preferred; current AL driver’s license, eligibility for coverage under center liability insurance and individually held vehicle liability insurance and clear background check.
