The Multipurpose Senior Services Program (MSSP) is a program of the State of California that aims to prevent or delay nursing home placement of low income, frail clients who are 65 years of age or older. The Sourcewise Care Management Department is seeking a Social Work Care Manager to work collaboratively with a team of social workers and nurses to provide temporary per diem help with enrolling clients and doing monthly check in calls.

The Sourcewise Care Management department administers the California Multipurpose Senior Services Program (MSSP), which aims to prevent or delay nursing home placement of frail clients who are 65 years of age or older.

Job Responsibilities:

• Conduct biopsychosocial assessments to identify care needs and client concerns
• Review program requirements and obtain client consent for participation
• Formulate care plans through assessment and consultation
• Monitor participants’ needs and care plan provisions
• Meet program and documentation requirements for home visits and telephonic contact

Education/Experience:

Master's Degree in social work, psychology, counseling, rehabilitation, gerontology, or sociology and 1 year of working with older adults

or Bachelor's Degree in social work, psychology, counseling, rehabilitation, gerontology, or sociology and 2 years of experience working with older adults

Qualifications:

• Valid CA driver’s license
• Experience with Case Management and Care Planning
• Knowledge of client interviewing and biopsychosocial assessment
• Knowledge of Elder Abuse Reporting requirements
• Ability to relate to frail, low income seniors from diverse cultures and ethnicities
• Ability to establish and maintain productive and collaborative working relationships
• Effective oral and written communication skills
• Ability to maintain accurate and up-to-date client records
• Effective time management skills

Job Type: Temporary

Salary: $32.00 - $36.00 per hour
