Essential Functions:

• Comply with state practice act and/or American Occupational Therapy Association guidelines
• Coordinate care and relay information to Physicians, patients and family members along with the Home Health & Hospice interdisciplinary group in a timely and effective manner
• Deliver Occupational Therapy modalities appropriate to patients clinical status and needs
• Assess the patient’s need for durable medical equipment as well as other adaptive therapy
• Assess patients pain and utilize pain management that includes but is not limited to Occupational Therapy
• Provides, modifies or discontinues Occupational Therapy modalities based upon ongoing assessment/reassessment of patients clinical status and in accordance with physicians orders
• Participates in interdisciplinary team conferences in accordance with Agency policy

Qualifications:

• Must be a graduate of an approved/accredited occupational therapy program
• Must have a current license to practice physical therapy in state of employment
• Current driver's license and vehicle insurance, and access to a dependable vehicle
• Current BCLS certification
• Current health certificate/physical examination and TB testing
• Prior Home Health Experience preferred but not required
• Ability to pass criminal background check, drug/alcohol screening, and OIG screening
