SUMMARY
In conjunction with the Wellness Director, the Wellness Nurse supports, and assists to assure resident’s cleanliness, nourishment, rest and activity in the Community in a manner conducive to the resident’s comfort and safety, The Wellness Nurse performs the duties of medication ordering, set-up and passes medications to assigned residents in accordance with the Company’s established procedures and as may be directed by the Wellness Director, Executive Director, Pharmacy Consultant and/or Medical Consultant. The Wellness Nurse performs the duties of monitoring care alerts and 24 hour report for content and accuracy, reports and follows-up on all risk management reports, and monitors residents post incident. Ensures all these systems are in place and function well.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
  • All aspects of Medication Management preserving safety, dignity, and self-respect of all residents at all the time
  • Orders medication under the direction of the resident’s physician when needed upon receipt of physician’s order; receives all medications of new residents and logs them into the appropriate system used by the community
  • Ensures that the Wellness Center and Medication Carts are appropriately stocked, cleaned, and organized
  • Notifies the resident and/or responsible party, Wellness Director and Pharmacy when changes in prescriptions are made
  • Updates the resident’s “medication record’” as necessary and monitors the medication/treatment records for accuracy and completeness
  • Count controlled substance/narcotics in ALIS at the beginning and end of your shift. Report any discrepancies to the Wellness Director
  • Conducts ongoing healthcare oversight for each resident to monitor and record any changes in the health of the resident will report any the above changes to the Wellness Director. Under the direction of the Wellness Director, the Wellness Nurse will notify the resident and/or the resident’s responsible party (if applicable) and interact with the resident’s physician when necessary.
  • Monitor the DARE report, care alerts, 24 Hour Report, neighborhood binders and acuity documentation for accuracy and follow up.
  • Assists the Wellness Director with the Assessments and its components (state specific), with the monthly service plans, update the Emergency Packs in ALIS for each resident, and oversees that all call signals are answered promptly
  • Notifies the Wellness Director of any equipment and supply needs while maintaining the Resident Care budget
  • Contributes to maintaining survey readiness and ensures compliance with state regulations
  • Maintains clear communication with Resident Assistants to monitor procedures and that the delivery of care is effective
COMPANY WIDE RESPONSIBILITIES
  • Supports a dignified and caring atmosphere with residents, residents' families, visitors and staff.
  • Serves in a supervisory role for Resident Caregivers during shift
  • Protects privacy and confidentiality of information pertaining to the resident, team member, residence, company information and records.
  • Maintains a safe and secure working environment and practices safe working habits.
  • Complies with the attendance policy.
  • Maintains neat appearance, good personal hygiene and appropriate uniform.
  • Participates in training, in services and attends meetings when required.
  • Assists with special projects as assigned and performs other related duties.
QUALIFICATIONS

Prior to presence in the community, applicant must have completed a Post Hire Health Questionnaire upon offer of employment. A TB test result must be received with 7 days of employment. Previous TB test results cannot be older than 6 months; chest x-ray results 2 years. A Criminal Record Clearance is required prior to team member’s starting date.
EDUCATION and/or EXPERIENCE
Must be 18 years of age or older and have high school degree or the equivalent. Must have and maintain a current First Aid certificate. Must be licensed practical nurse (LPN) or registered nurse (RN) in the community’s state. Must have completed Health Screening Report and a Criminal Record Clearance. One year or more work experience in home health care, skilled nursing for the elderly or assisted living. Must have completed a Health Screening Report and a Criminal Record Clearance.
LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or team members of organization.
MATHEMATICAL SKILLS
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
REASONING ABILITY
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
CERTIFICATES, LICENSES, REGISTRATIONS
Licensed as a practitioner nurse (LPN) or registered nurse (RN). Must carry and maintain current CPR certificate.
OTHER SKILLS AND ABILITIES
Proficient with MSWord, MSExcel, MSPowerPoint, MSOutlook, and other systems knowledge is helpful. Basic typing skills essential.
PHYSICAL DEMANDS
Position requires the ability to stand, reach, bend/stoop/crouch, push/pull/lift/carry up to 50lbs, twisting/ turning and repetitive motion
PHYSICAL ABILITIES
Must have good vision, hearing and speech
WORK ENVIRONMENT
Primarily works in the community.
